SCOPE: These specifications cover the printing and binding of approx. 4,000,080 (includes 200 Blue Labels) forms booklets packing into approx. 10,527 cartons and shipping/mailing to approx. 3,122 destinations. Each forms booklet contains 16 pages requiring a vertical perforation on the fold through all eight (8) leaves along the entire bind side. In addition, contractor will have to create mailing and shipping labels; address, label and pack cartons; palletize, stage, and mail/ship via Parcel Select, Small Package Carrier, Motor Freight, and Air Freight (if req’d).
