Please see the attached Solicitation, the Statement of Work, Amendment 0001, and the Question and Answers document for complete details. The contractor will provide, on an as-needed basis, required laboratory testing for blood and urine specimens for specific job required physical examinations conducted by Health Services Division. Health Services Division will take laboratory specimens from approximately 10-15 persons per month. The contractor shall provide the COR a schedule of costs to the client for mutually agreed upon tests and services. The contractor shall not charge the client more than those costs on the schedule without prior approval from the client. All financial transactions are between the contractor and the client.
