Please read and follow all instructions provided in the RFQ letter and below. Failure to do so could result in your submission being excluded from evaluation. This is a Request For Quote (RFQ) to purchase GE HealthCare items. Please review all four attachments which include the RFQ letter, Attachment 1 Price Spreadsheet, Attachment 2 Brand Name Justification and Attachment 3 C-SCRM Questionnaire. Submissions must include the following in accordance the RFQ instructions: Technical volume to include all information requested in RFQ letter paragraph 3.1. Pricing Sheet, with all highlighted information filled out in accordance with RFQ letter paragraph 3.2. Must be provided in the format attached. C-SCRM Questionnaire, fully filled out in accordance with RFQ letter paragraph 3.3. Must be provided in the format attached. Any quotes received without the required documentation listed above and following the RFQ instructions may be considered unacceptable and not evaluated. Responses to this RFQ must be submitted by the due date on this posting via email to laura.farren@gsa.gov and victoria.howland@gsa.gov.
