This Failure Analysis (FA) for Damper Actuators is to provide government decision makers with sufficientlydocumented data to define and determine the failure mode and any contributions to the failure of damper actuators associated with the building automation system of building 21 and 22. To this end, the FA shall provide an objective, analytical analysis that optimally meets the identified needs. It will produce both a report of findings, as well as a recommended solution to failures. Ultimately the FA will be used as the primary supporting document in the preparation of a recommended repair decision provided to the Contracting Officer for the current O&M contract. These two buildings are located at Office Buildings 21 and 22, FDA White Oak Campus, Silver Spring, MD
