This announcement constitutes the only solicitation; quotes are being requested and a written solicitation will not be issued. (ii) Solicitation 36S79722Q0005 is issued as a Request For Quotation (RFQ). (iii) This solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 2022-04. (iv) This solicitation is 100% Service-Disabled Veteran Owned Small Business Set-aside. The NAICS code associated with this solicitation is: 519190. The size standard is: $30 million. (v) PBX Phone System, line items as follows: CLIN Description Qty Unit of Issue Unit Price Total Price 0001 PBX Phone System Operation and Maintenance Services in accordance with Statement of Work BASE PERIOD: 07/01/2022 06/30/2023 12 MONTHS 0002 PBX Phone System Operation and Maintenance Services in accordance with Statement of Work OPTION 1: 07/01/2023 06/30/2024 12 MONTHS 0003 PBX Phone System Operation and Maintenance Services in accordance with Statement of Work OPTION 2: 07/01/2024 06/30/2025 12 MONTHS (vi) See attached detail description of requirements STATEMENT OF WORK PBX PHONE SYSTEM STANDARD OPERATIONAL AND MAINTENANCE SERVICE MOVES, ADDS, CHANGES, DISCONNECTS (MACD) The Contractor shall provide all required moves, adds and change (MAC) services to operate, maintain, and perform station side level repairs to the telephone systems that make up the telephony system located at the Service Delivery Center (SDC) to ensure the PBX and Voice over Internet Protocol (VoIP) equipment operates with a 99.999 percent availability. The Contractor shall perform system checks and working file backups on all Service Delivery Center (SDC) equipment to include all ancillary systems relating to the PBX (Auto-Attendant, Voicemail, Call-Accounting, and Telecommunication Management System [TMS]) as required to protect the Government from loss of systems and data. The Contractor shall place all backups on the Department of Veterans Affairs (VA) network attached storage (NAS). The Contractor shall complete full system backups on all PBX and any ancillary systems every 30 days. The Contractor shall provide an Escalation List to the COR that provides a list of personnel and their contact information for notifications for emergency, routine maintenance, and follow-on service to enable response within the prescribed time frames and conditions as specified in this TO. The Contractor shall update the Escalation List as required and provide this information to the COR. The Contractor shall comply with the national codes and telecommunications industry standards and practices. MOVE, ADD, AND CHANGE (MAC) The Contractor shall relocate, add, repair, or perform changes of the telephone equipment that are part of the Service Delivery Center (SDC), to include the infrastructure, within the facilities. The Contractor shall initiate services within one business day after receipt of a MAC request and complete the MAC work within two business days of initiation. Historically, there have been approximately 15 to 20 MAC requests per MONTH. The Contractor is not responsible for consumable items needed to perform MAC work. The VA will provide all consumables needed to perform any MAC activities at the SDC and affiliated locations. The contractors will use the VA s National Service Desk (NSD) ticketing system to receive, log status changes and work progress, and close tickets for all MAC work. The tickets Notifications are received through the Contractor s VA issued Outlook account. The tickets can be accessed with a VA issued account to the CA service Desk Manager Website. ON-SITE SUPPORT The Contractor shall be trained and certified on all the installed PBX system types, and telephony peripherals at the Service Delivery Center (SDC) such as NEC SV9500 and the AVST CallXpress. The Contactor shall provide the names of all on-site technicians, their OEM certifications and training records to the COR. The Contractor shall request through the VA National Service helpdesk Tier III Department of Veterans Administration Unified Communication Infrastructure (UCI) support for issues requiring elevated diagnostic testing and technical problem assistance. As additional SDC equipment is installed, the Contractor shall obtain manufacturer certification for that additional equipment within 30 days of the installation of the additional equipment. Any additional SDC equipment installed or replaced at the facilities, will be considered part of the overall SDC system that the contractor is required to support as part of this effort. The Contractor shall provide remote and on-site support as needed at the Service Delivery Center (SDC), located at Hines SDC, IL. during normal duty hours, VA hours of operation are 0600A.M. to 6:00 P.M. Monday through Friday excluding Federal holidays for no more than 40 hours per month. 40 hours per month enables the contractor to monitor the work order mail group remotely and make necessary programming changes via MA4000 and the UM8700 software for mailbox issues. The contractor shall manage their time as the work orders dictate. If a telephone move is required, the technician will be required to be onsite to make that adjustment in the server rooms. The contractor may be required to perform work after hours in the event of an emergency, but no hours above the maximum of 40 hours per month will be required. Weekly time sheets shall be provided to the COR. EMERGENCY MAINTENANCE CALL SUPPORT The Contractor shall begin remote failure remediation within 30 minutes after VA notification to the Contractor s service desk or representative. If the failure cannot be completely resolved remotely within two hours after VA initial notification to the Contractor s service desk, the Contractor shall be on-site within four hours of the initial call to the Contractor s service desk. That is, the Contractor shall provide 2-hour remote failure remediation, and 4-hour on-site remediation, if necessary, 24 hours a day, 7 days a week to all emergency maintenance calls. Once repairs are initiated, work shall continue until completed. Emergency calls are invoked by either failure in a critical care area, or by type and scope of a failure. Critical care areas warranting emergency response within a VA facility setting include Medical Intensive Care Units, Surgical Intensive Care Units, Coronary Care Units, Operating Room Suites, cardiac arrest team centers, intensive care units, emergency departments, operating rooms, community living centers, CBOCs, and any other areas deemed critical care areas by the COR, FCIO, or designees. An emergency maintenance call shall be deemed appropriate when a failure involves a trunk group, 100 ports or more, a port shelf, or cabinet. In addition, the failure of a common control unit; power supply; signal generating device; or attendant console or attendant telecommunication devices and or any computer, or any other area/device the COR, FCIO, or designees deems critical shall also be deemed as warranting an emergency maintenance call. Historically, there has been one to two emergency maintenance calls per year. ROUTINE MAINTENANCE CALL SUPPORT A fault event that does not qualify as emergency shall be addressed as a routine maintenance call. Historically, there has been one to two routine maintenance calls per year. After receipt of a routine maintenance call, the Contractor shall initiate troubleshooting and repair within 24 hours. Once repairs are initiated, work shall continue until completed. Normal maintenance items that require system shutdown shall be scheduled after normal VA business hours. The Contractor shall coordinate the date/time with the COR, FCIO, or designees seven days prior to the shutdown. TROUBLESHOOTING AND REPAIRS Any problem with the SDC shall be isolated, repaired or escalated to VA UCIs by the Contractor. The Contractor shall trouble shoot the hardware, software, communications/ancillary equipment, and infrastructure to determine the best method of repair. If problems are found, the Contractor shall contact the COR, FCIO, or designees and continue corrective measures until the problem is resolved. The Contractor shall document the repair in VA s NSD work item ticket system including a description of actions taken and resolution. The VA shall provide all equipment necessary to troubleshoot and complete repairs. Additionally, if any station/trunk card, telephone instrument, power plant battery backup system, telephony server, or any other system component fails, the VA shall replace the failed equipment/software with new/refurbished, VA-provided equipment/software. Historically, there have been approximately two problems each year where a line card and digital telephones needed replacing. All quotes submitted must include unit and extended prices for all CLINS listed in the schedule. Alternate schedule formats are permitted but must include pricing for all CLINS. Quoters must comply with the requirements at VAAR 852.219-10 and be registered with the Vendors Information Pages databases (www.VetBiz.gov). The Government will evaluate past performance only using information in the quoters past performance record found on Government databases, such as Federal Awardee Performance and Integrity Information System (FAPIIS) and Contractor Performance Assessment Reporting System (CPARS). No past performance information or references are required for quote submission. All prices submitted will be evaluated for fairness, reasonableness, and balance. Unbalanced pricing exists when, despite an acceptable total evaluated price, the price of one or more line items is significantly over or understated as indicated by the application of price analysis techniques. Any quote deemed unbalanced will not be acceptable. (ix) The provision at FAR 52.212-2, Evaluation-Commercial Items applies to this acquisition. The following is addendum to FAR 52.212-4. E.O. 14055, November 18, 2021 Nondisplacement of Qualified Workers: (a) The contractor and its subcontractors shall, except as otherwise provided herein, in good faith offer service employees (as defined in the Service Contract Act of 1965, as amended, 41 U.S.C. 6701(3)) employed under the predecessor contract and its subcontracts whose employment would be terminated as a result of the award of this contract or the expiration of the contract under which the employees were hired, a right of first refusal of employment under this contract in positions for which those employees are qualified. The contractor and its subcontractors shall determine the number of employees necessary for efficient performance of this contract and may elect to employ more or fewer employees than the predecessor contractor employed in connection with performance of the work solely on the basis of that determination. Except as provided in paragraph (b), there shall be no employment opening under this contract or subcontract, and the contractor and any subcontractors shall not offer employment under this contract to any person prior to having complied fully with the obligations described in this clause. The contractor and its subcontractors shall make an express offer of employment to each employee as provided herein and shall state the time within which the employee must accept such offer, but in no case shall the period within which the employee must accept the offer of employment be less than 10 business days. (b) Notwithstanding the obligation under paragraph (a) above, the contractor and any subcontractors (1) are not required to offer a right of first refusal to any employee(s) of the predecessor contractor who are not service employees within the meaning of the Service Contract Act of 1965, as amended, 41 U.S.C. 6701(3), and (2) are not required to offer a right of first refusal to any employee(s) of the predecessor contractor for whom the contractor or any of its subcontractors reasonably believes, based on reliable evidence of the particular employees' past performance, that there would be just cause to discharge the employee(s) if employed by the contractor or any subcontractors. (c) The contractor shall, not less than 10 business days before the earlier of the completion of this contract or of its work on this contract, furnish the Contracting Officer a certified list of the names of all service employees working under this contract and its subcontracts during the last month of contract performance. The list shall also contain anniversary dates of employment of each service employee under this contract and its predecessor contracts either with the current or predecessor contractors or their subcontractors. The Contracting Officer shall provide the list to the successor contractor, and the list shall be provided on request to employees or their representatives, consistent with the Privacy Act, 5 U.S.C. 552a, and other applicable law. (d) If it is determined, pursuant to regulations issued by the Secretary of Labor (Secretary), that the contractor or its subcontractors are not in compliance with the requirements of this clause or any regulation or order of the Secretary, the Secretary may impose appropriate sanctions against the contractor or its subcontractors, as provided in Executive Order (No.) 14055, the regulations implementing that order, and relevant orders of the Secretary, or as otherwise provided by law. (e) In every subcontract entered into in order to perform services under this contract, the contractor will include provisions that ensure that each subcontractor will honor the requirements of paragraphs (a) and (b) with respect to the employees of a predecessor subcontractor or subcontractors working under this contract, as well as of a predecessor contractor and its subcontractors. The subcontract shall also include provisions to ensure that the subcontractor will provide the contractor with the information about the employees of the subcontractor needed by the contractor to comply with paragraph (c) of this clause. The contractor shall take such action with respect to any such subcontract as may be directed by the Secretary as a means of enforcing such provisions, including the imposition of sanctions for noncompliance: provided, however, that if the contractor, as a result of such direction, becomes involved in litigation with a subcontractor, or is threatened with such involvement, the contractor may request that the United States enter into such litigation to protect the interests of the United States. Incumbent contractor: MIS Choice 300 N Martingale RD Suite 250 Schaumburg IL 60173 Nicolo Gazzano President The Government will award a contract resulting from this RFQ to the responsible quoter who meets the set-aside criteria of 852.219-10, and whose quote conforming to the RFQ is the lowest total evaluated priced quote with acceptable past performance. The Government intends to award without discussions. The Government will calculate the quote s total evaluated price (TEP). TEP will be calculated by confirming the quote s unit price multiplied by the quantity equals the extended price listed. If the calculation does not equal the extended price, the Government will correct this mathematical error and adjust the quoter s extended price to reflect the correct calculation of unit price multiplied by quantity. The Government will then evaluate the option pricing for the option at FAR 52.217-8 by using the unit prices found in the last option CLIN 0003 to calculate the option FAR 52.217-8 extended pricing for this 6-month option. This price will be included in the evaluation only and not included on any resulting contract award. The final extended price of all CLINS added together plus the calculated price of the FAR 52.217-8 option will determine the quoter s TEP. Quote TEP will then be ranked from lowest to highest. Only the lowest quote will proceed to the past performance evaluation. The Government will use Government past performance resources to assign the quote with one of the following ratings: Acceptable: Based on the quoter s performance record, the Government has a reasonable expectation that the quoter will successfully performance the required effort, or the quoter s performance record is unknown. Unacceptable: Based on the quoter s performance record, the Government does not have a reasonable expectation that the quoter will be able to successfully perform the required effort. If the lowest priced quote is evaluated to have acceptable past performance, then that quote will be selected for contract award. If the lowest priced quote is evaluated to have unacceptable past performance, then the next lowest quote will move to the past performance evaluation step until a quoter is identified with acceptable past performance. The Government intends to make one contract award without discussions to the lowest priced quote with acceptable past performance. (x) Offerors must include a completed copy of the provision at FAR 52.212-3, Offeror Representations and Certifications-Commercial Items when submitting their quote.
