PRE-SOLICITATION NOTICE: The VA National Acquisition Center intends to issue Request for Proposal (RFP) 36E79721R0026 for an unrestrictive procurement for Melatonin Tablets, for the Department of Veterans Affairs (VA), Department of Defense (DOD), Bureau of Prisons (BOP), and Indian Health Service (IHS). The VA will award one contract to the responsible offeror who will be able to provide an uninterrupted source of supply for the contracted items. The contract period will be for one year plus four pre-priced one-year option periods. The contract items will be distributed through the VA s and DOD s respective Pharmaceutical Prime Vendor Programs. Offerors must state the exact name (name that appears on label) by which the drug will be supplied and have a unique NDC number or UPC code. Solicitation 36E79721R0026 will be electronically issued in commercial item format in accordance with FAR Part 12 on or about March 31, 2021, with a tentative closing date of April 14, 2021. The NAICS code for this procurement is 325412. Any future amendments and other miscellaneous documents will be available electronically for download at the following URL: http://www.beta.SAM.gov. No paper copies of the solicitation will be available, and no telephone requests for paper copies of the solicitation will be accepted. The estimated annual requirements are as follows: ITEM DESCRIPTION PACKAGE SIZE (BOTTLES) TOTAL ESTIMATED ANNUAL USAGE (BOTTLES) 1 Melatonin 1mg Tablets 90 20,184 2 Melatonin 3mg Tablets 60 1,456,724 3 Melatonin 5mg Tablets 90 275,842 The offeror may instead of an NDC, propose using a unique UPC code that meets the barcode/UPC requirements that will be listed in the RFP. Product must be certified by a company such as USP (USP Verified), ConsumerLab (CL Approved), NSF International (NSF Mark), National Products Association (NPA), (TruLabel) or other which provides a physical inspection of the manufacturing site and assurance that the product was properly manufactured, that it contains the ingredients listed on the label in the stated amounts, and that does not contain harmful contaminants. Certification must be maintained throughout the contract period. The certifications must be submitted by the due date for offers. Failure to provide this certification shall be rejected and shall receive no further consideration. Line items 1-3 are unit of use. All unit of use packaging must contain a child proof closure (safety cap) and a bottle with a minimum required size of 120cc volume. The safety cap must not exceed the diameter of the bottle. The bottle must have a cylindrical body and minimum of 5.75 circumference. The bottle must fit a label with a dimension of 5.25 x 2 . The 0.5 additional space is required to keep the bar code exposed and/or allow for the display of the expiration date and lot number. Product Samples. Offerors shall provide one sample (one bottle) for line items 1, 2 and 3. Samples will not be returned. (See FAR Clause 52.212-1(c)). The samples must be submitted by the due date for offers. Any offer whose proposed samples fail to meet the minimum requirements shall be rejected and shall receive no further consideration.
