CSAS has solicited for this requirement under Solicitation ID 36C79121R0008 and is currently evaluating proposals. Due to delays, VA intends to award a sole source contract to the incumbent vendors to bridge the services gap while evaluations are finalized. The NAICS Code is 339112; size standard of 1000 Employees. The Product or Service Code (PSC) is 6515. VA and CSAS find no other potential sources to fulfil this requirement as the current vendors are providing patient care using proprietary services and equipment. All other potential sources have been Identified and have submitted proposals to solicitation 36C79121R0008 and are currently being evaluated. The contracting officer will place a sole-source award with Medtronic Care Management Services, LLC, a large business, SAM UEI: GFUQJV1ETL89 of Chanhassen, MN and Cognosante MVH, LLC, a large business, SAM UEI: XDDKMXTVJSN8. Each IDIQ contract will have a $10,000 Minimum Guarantee and will include a six-month base plus four six-month option periods.
