PURPOSE: This is a Request for Information(RFI)/Sources Sought announcement only. This is not a solicitation announcement. The NCO 15 Consolidated Mail Outpatient Pharmacy (CMOP), Contracting office is conducting market research only to gain knowledge of potential qualified sources (including VOSBs & SDVOSBs) and is issuing this Sources Sought Notice for the following pharmaceutical: CIPROFLOXACIN 400 MG/200ML-D5W, NDC #25021-0114-87, quantity 3,087 Cartons (24 pouches per carton). Alternate NDCs will be considered as long as it meets the pharmaceutical equivalent and is approved by the CMOP. Product requirements: Must have expiration date of 18 months or longer. One time delivery, with the fewest lot numbers as possible. To be considered, vendors must: Be registered in Sam.gov under NAICS code 325412 (Pharmaceutical Preparation manufacturing) Provide Pedigree for pharmaceuticals Must be a Manufacturer of the item or; an Authorized Distributor of the item or; an Authorized Distributor of a Reseller who is an Authorized Distributor of the Manufacture of the item and can provide a Letter of Authorization from the Manufacturer showing them as an authorized distributor Offers are not being solicited at this time. The Government does not intend to award a contract on the basis of this request for information. Vendors are required to address the following questions to be considered a viable source for future needs: Company name Company s Unique Entity Identifier (UEI)# Type of Business (large, small, VOSB, SDVOSB etc.) Point of Contact: Name Phone Number Email Address NDC number you can supply with country of origin DELIVERY, what is estimated ARO (after receipt of order) days Interested sources may respond to this notice via email to Brigitte.kidder@va.gov, by Wednesday, July 10th, 2024 @ 10am CDT.
