STATEMENT OF WORK U.S. Department of Veterans Affairs Consolidated Mail Outpatient Pharmacy (CMOP) 4136 Carolina Commerce Pkwy Ladson, SC 29456 Background: The Charleston-VA CMOP requires the rental of four(4) Automated External Defibrillator s (AED) at the new facility at Carolina Commerce Parkway, Ladson SC. Project Scope: The contractor will be required to provide four(4) AED s which should include: Adult pads Replacement batteries Replacement Pads AED rescue pack Cabinet with Alarm and wall sign Vendor shall provide yearly inspections for each AED unit Inspection of AED s will be held within 30 business days of annual expiration. Any software updates Three(3) Basic Life Support and AED annual training sessions every 2 years or as required for individuals to maintain certification for: 10-12 Individuals per class One(1) session for day shift employees, One(1) session in the early afternoon for nightshift employees One(1) mid-day session to cover any remaining staff, if necessary. Training will be held in-person within 30 business days of expiration of Basic Life Training certificates Bldg Security Requirements: All workers to provide proper identification in the form of a pictured state issued driver s license or ID card. Sign-in at front office and complete COVID-19 screening, as applicable before entering bldg to begin work. All workers are required to check-out upon departure. Escorts are provided and all areas are subject to video surveillance.
