The Department of Veterans Affairs, National Contracting Office 15, CMOP Division intends to release a requirement to procure four (4) medical and surgical supply listed below for delivery to the CMOP facility which is located at: Department of Veterans Affairs VA CMOP Leavenworth - 760 5000 S. 13th Street Leavenworth, KS 66048-5580 RFQ: 36C77023Q0039 SET ASIDE CATEGORY: SDVOSB set-aside, Gray Market requirements apply PRODUCT CODES: 6515, Medical and surgical instruments, equipment, and supplies NAICS CODES: 325413, In-vitro diagnostic substance manufacturing ESTIMATED ISSUE DATE: 11/15/2022 ESTIMATED RESPONSE DUE DATE: 11/22/2022 DELIVERY TIME FRAME: 10 days (ARO) after receipt of order All responsible sources may submit a quotation, which if received timely, shall be considered by this agency. Responses must be concise and be specifically directed to the requirement reference above. It is the offeror s responsibility to monitor SAM.GOV for changes or amendments. Offeror shall provide OEM authorized dealer, distributor, or reseller documentation. Deviation to policy allows non-manufacturing vendors selling medical surgical supplies to satisfy the Gray Market Prevention language by providing letter of authorization from a manufacturer s authorized wholesaler/reseller via an authorization directly from the manufacturer. Vendors that fail to submit a copy of their OEM authorized dealer or distributor documentation shall be deemed non-compliant. All solicitation packages will be submitted via email. 1.
