The Department of Veterans Affairs, Network 15, Consolidated Mail Outpatient Pharmacy (CMOP) Contracting Office has a requirement to procure the items listed below for delivery to Hines CMOP to replenish stock: LINE ITEM NDC DESCRIPTION PKG QTY UNIT OF MEASURE 1 11704-620-01 Atropine & Pralidoxime Chloride Injection (DUODOTE), 30 per box, Expiration date: 2025 or beyond 30 246 BX The Product Code for this procurement is 6505 (Drugs & Biologicals) and the NAICS code for this solicitation is 325412 (Pharmaceutical Preparation manufacturing). All responsible sources may submit a quotation, which if timely received, shall be considered by this agency. Submit the RFQ (solicitation form 1449), and quote spreadsheet via email (preferred) to: Brigitte.Kidder@va.gov. Solicitation package 36C77023Q0016 shall be posted on Contract Opportunities on or about November 7, 2022, and will close on November 15, 2022, at 12:01pm CST. These dates are subject to change.
