The Department of Veterans Affairs, Network 15, Consolidated Mail Outpatient Pharmacy (CMOP) Contracting Office has a requirement to procure the items listed below for delivery to Hines CMOP Emergency Pharmacy Services (EPS) to replenish stock: LINE ITEM NDC DESCRIPTION PKG QTY UNIT OF MEASURE 1 43063-0547-01, or generic equivalent (with EPS approval) Ciprofloxacin 500mg tablets, 100 per bottle, Expiration date: 2025 or beyond and least amount of lot numbers 100 60,766 bottles BT The Product Code for this procurement is 6505 (Drugs & Biologicals) and the NAICS code for this solicitation is 325412 (Pharmaceutical Preparation manufacturing). All responsible sources may submit a quotation, which if timely received, shall be considered by this agency. Submit the RFQ (solicitation form 1449), and quote spreadsheet via email (preferred) to: Brigitte.Kidder@va.gov. Solicitation package 36C77023Q0015 shall be posted on Contract Opportunities on or about November 30, 2022, and will close on December 5, 2022, at 5pm CST. These dates are subject to change. No solicitation packages will be mailed. Responses must be concise and be specifically directed to the requirement referenced above. It is the offeror s responsibility to monitor Contract Opportunities (Sam.gov) for changes or amendments. This requirement will be solicited as unrestricted. The Drug Supply Chain Security Act (DSCSA) October 2013 is applicable to this procurement.
