STATEMENT OF REQUIREMENTS (SOR) Illinois CMOP Automatic Capping Machine Delivery and Placement BACKGROUND & INTRODUCTION The Great Lakes Consolidated Mail Out-Patient Pharmacy (GL-CMOP) located at 5th and Roosevelt Building 37 NW, Hines, IL 60141 is a provider of outpatient pharmaceutical services dispensing approximately 100,000 prescriptions daily directly to patients by mail. The Pharmacy operates across two (2) shifts from 5 am to Midnight (12 am) with a staff compliment of approximately 300 people. OBJECTIVE The GL-CMOP has need for a state-of-the-art capping machine with full support to replace the failing and unsupportable capper (operating at this location), which is not capable of supporting Tablet Dispensing System (TDS) industry packaging operations at its full output capacity. Replacement of the failing capper is necessary for the GL-CMOP to sustain its day-to-day production operations in concert with the TDS to dispense an average of 16,000 prescriptions per day and four million annually. This replacement state-of-the-art capping machine affords quickly supportable, reliable, and maintainable hardware. SCOPE The purpose of this contract is to purchase, deliver, transport, assemble, put in place, test, train (operator and maintenance), and commission a new capping machine that is capable of expulsing faulty caps, placing, and tightening serviceable caps of various sized pharmaceutical bottles to adjustable torque settings. The capping machine must be able to support current TDS production operations. The Contractor s representative shall schedule all work with the Contracting Officer s Representative (COR). The COR is designated at the time of award. If on-site work is necessary, then all contract personnel shall present a government issued picture ID and sign in to receive a Contractor identification badge for escort before any work is performed. Contractor shall perform all delivery, assembly, and associated work according to federal, state, municipal, local guidelines, and ordinances as applicable. All safety practices prescribed by these agencies shall be followed. PERFORMANCE REQUIREMENTS Contractor requirements are as follows: Capper is a stand-alone system. Current Capping Machine specifications needed as minimum requirements: The capper shall support 120cc (93mm in height, 49mm in width) and 250cc (109.5mm in height, 63mm in width) bottles with a 35mm diameter bottle (PET, and PVC) throat having an 8mm throat area. Single step that torques the cap/s to the required specification. Capable to set torque settings from 12 in per lb. 17 in per lb. Capable of processing no less than 2000 bottles per hour (bph). Places caps on bottles within specified torque settings. Contains capping head safety features to expel faulty caps that remain stuck in the chuck. Capping head constructed with food grade materiel and with a griping system. Capable of simultaneous capping with both 120cc and 250cc bottles. Capable of supporting caps with a diameter of 45mm x 18mm (height). Capable to run on 208V single phase electric power. A warning light system. Cap elevator feed, and automatic feed. Provide on-site, stand-by, and support services during assembly, installation, testing, and through final acceptance. Provide operation and maintenance training addressing the structure, function, and operation of all components as well as the interaction between components. Provide one copy of the Operations and Maintenance Manual (may also be an electronic copy) including a preliminary Operations and Maintenance manual with schematic drawings down to component level (in English) prior to testing and final acceptance. Capable to integrate into current conveyor systems. GOVERNMENT PROVIDED DELIVERABLES The Government shall provide the following conditions, equipment, access, and facilities for the Contractor to complete all the performance requirements to the performance standards in this SOR. Capper fits on the GL-CMOP production floor within a footprint noted below as follows: 5 (width) x 11.5 (length) x 14 (height) with working direction and working height occupying current floor-space footprint. Provide sufficient access to the facility as required to perform the on-site work. Operators, orders, bottles, and inventory as required for testing, mock shifts, and final commissioning. Provide and ship (round-trip) test bottles, caps, and bottle pucks for pre-shipment testing (all Government provided testing supplies need to be returned). Escorted access to the GL-CMOP during hours of operation. COR approval is needed for access outside of normal GL-CMOP operating hours. Security for the installation areas provided by qualified security personnel. Provide a clean, lighted, and safe working area. Broom cleaned installation areas. Dock doors as required for unloading tractor trailers at the installation areas. Provide GL-CMOP operators and maintenance personnel to attend any required training. Provide operators and maintainers to provide regularly scheduled care and maintenance. PERFORMANCE STANDARDS A. Performance testing of the capper shall meet all listed performance requirements prior to acceptance. B. Proper adherence to all safety procedures is required prior to, and during work performance. When on-site work is required, then the contractor shall furnish all personnel with any required personal protective equipment (PPE). The contractor shall safeguard the public and government personnel, property, materials, supplies, and equipment exposed to its operations and activities, and avoid interruptions of government operations while performing SOR requirements. DELIVERABLES The contractor shall provide a detailed report (inspection report included) with progress reports of all delivered, installed, and tested equipment commissioned to the COR GL-CMOP at 5th and Roosevelt, Building 37 NW Dock 18, Hines, IL 69141-3030 at least twenty-four hours before acceptance testing begins. The Inspection Report shall include a list of equipment and components installed and assembled documenting the condition of systems inspected and recommendations for repairs and maintenance. POC contact information provided from the CO. Contractor shall provide a list of names with who are to work on-site and at a minimum of one week prior to on-site work, installation, and repairs to facilitate security pass approval. QUALIFICATIONS Contractor shall provide personnel who have the knowledge, experience, qualifications, and working experience to perform the functions outlined in this SOR. Additionally, electricians shall be certified and meet all local ordinance, code, and permit requirements. CONTINUITY OF FACILITIES OPERATION A. Smoking Policy (includes all tobacco products) Vendor is advised that the Government has placed restrictions on the smoking of tobacco products in Government facilities. Vendor s employees and visitors are subject to the same restrictions as Government personnel. Smoking is permitted only in designated smoking areas. B. Safety Requirements The vendor shall comply with all local, state, federal laws, and safety regulations. Compliance with OSHA and other applicable laws and regulations for the protection of contractor s employees (exclusively the contractor s obligation), and the Government assumes no liability or responsibility for the contractor s compliance or non-compliance with such responsibilities. The vendor shall provide their employees with all necessary personal protective clothing and equipment (PPE). The Contracting Officer or his/her designee will notify the contractor of any noncompliance with the foregoing provisions and action to be taken. After receipt of such notice, then the contractor shall immediately correct the conditions to which attention has been directed. Such notice (when served to the contractor or his representative) at the site of the work shall be deemed sufficient for the aforementioned purpose. If the contractor fails or refuses to comply promptly then the Contracting Officer may issue an order stopping all or any part of the work and hold the contractor in default as provisioned elsewhere in this contract. C. Weapons, Firearms, and Ammunition Contractor s employees are prohibited from possessing weapons, firearms, or ammunition upon themselves or on Government property while at GL-CMOP. D. Fire Prevention and Safety Contractor s employees shall comply with the Occupational Safety and Health Act (OSHA) and the Fire Prevention and Health Program. E. Standards of Conduct The Contracting Officer or COR shall refer to the Contractor for immediate action (including, but not limited) to remove any contract personnel from the work site whose personal or professional conduct jeopardizes or interferes with the regular and ordinary operations of the GL-CMOP facility. Breaches of conduct include but are not limited to intoxication or debilitation resulting from drug use; theft; dereliction; negligence in performing tasks or other conduct resulting in complaints by staff or management. F. Illegal Substances Illegal substance possession, use, or distribution will not be tolerated on any government installation, including the GL-CMOP. This includes prescribed medical marijuana. The contractor shall ensure its employees who have access do not possess, use, or distribute illegal substances. All vendors are required to adhere to all local, state, and federal laws concerning possession and/or use of illegal substances. Vendors found to be in possession of illegal substances will be barred from the installation and subject to arrest and prosecution by federal and local authorities. G. Parking It is the responsibility of the Contractor to park in the appropriate designated parking areas. The VA will not validate or render reimbursement for parking costs or violations incurred by the Contractor under any conditions. H. Interference to Normal Functions The Contractor may be required to interrupt work at any time so as not to interfere with the normal function of the facilities, fire protection systems, facility personnel, and equipment. I. Qualification of Offerors Quotations will be considered only from offerors who are regularly established in the business called for and who (in the judgment of the Contracting Officer) are financially responsible and able to show evidence of their reliability, ability, experience, equipment, facilities, and personnel directly employed or supervised by them to render prompt and satisfactory service, and adequately trained to perform as required in this SOR. Cell Phone Use - Cell phone use is not permitted on production floors. Contract personnel are required to adhere to local facility policies regarding cell phone use and electronic devices.
