Page 2 of 2 PURPOSE: This is a Request for Information/Sources Sought announcement only. This is not a solicitation announcement. Requests for a solicitation will not receive a response. The NCO 15 Consolidated Mail Outpatient Pharmacy (CMOP) Contracting is issuing this Request for Information (RFI) to locate potential sources to provide Automated Bags for Mail Out Pharmacy Prescriptions supporting the Charleston CMOP located in Charleston South Carlina. The requirement is for 5928 Cases or 8,892,000 bags per year. The requirement will include; 1) A base period plus 4 one(1) year options. 2) Will be use a Brand Name or Equal Equivalent to fulfill the need. SUMMARY: The NCO 15 CMOP Contracting is conducting market research to gain knowledge of potential qualified sources that can meet the requirements. Offers are not being solicited at this time. The Government does intend to award a contract on the basis of this request for information. Please answer all of the following questions. Is your company a Manufacturer of Bags that could meet this need? Yes or NO If Yes, IAW FAR Guidance, are they manufactured in the USA? Yes or NO If they are not manufactured in USA, what Country are they manufactured in? ________________________ If Not the Manufacturer, can you show that you are an Authorized Distributor of a Manufacturer? Yes or No Where are the bags manufactured? USA or Other. If other where? __________________ Any responses to this RFI must include answers to all of the above questions in order to adequately assess the capabilities of the marketplace in relation to the requirements of this procurement.
