OFFEROR READ THIS INFORMATION FIRST BEFORE SUBMITTING AN OFFER This is a combined synopsis/solicitation for commercial products and commercial services prepared in accordance with the format in Federal Acquisition Regulation (FAR) subpart 12.6, Streamlined Procedures for Evaluation and Solicitation for Commercial Products and Commercial Services, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation and a Request for Quote (RFQ) is being requested. Solicitation Number: 36C26324Q0206 The solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular (FAC) 2024 dated 11/14/2023 This is an unrestricted solicitation meaning all business size may respond. At the time of submission of offer and prior to award, the offeror shall be active in the System for Award Management (SAM) reflecting 621111 Offices of Physicians (except Mental Health Specialists). The PSC Code is Q505 Medical Gastroenterology. The VA anticipates the award of one (1) Indefinite Quantity Contract (IDC) for On-Site Gastroenterology Physician and Nurse Practitioner Services to support the Des Moines IA VA Health Care System located at 3600 30th Street, Des Moines IA 50310. See Performance Work Statement and Price/Cost Schedule for specific requirements and qualifications. The period of performance is from April 1, 2024, to March 31, 2025. The contract is required to provide the following services: Gastroenterology Clinic (Nurse Practitioner), Four Hour: Four (4), eight-hour clinics per week, (Estimated 32 hours/week, 112-128 hours/month, 1664* total hours/year (12) months). Nurse Practitioner minimum. *10 Hours of VA Mandatory Training included in total. Hepatology Clinic/E-consults (Gastroenterologist), Four Hour: Two four-hour clinics per month, (estimated 8 hours/month, 132* hours/year (12) months). Board Certified Physician minimum. *30 Hours of VA Mandatory Training (Three Physicians at 10 hours each) included in total. Inpatient/Phone Consult/Question Only: Inpatient/Phone Consult/E-consults Only consultation during business hours (estimated 7-10/ month, 100/ year (12) months) GI EGD Procedure: Esophagogastroduodenoscopy (EGD) procedures (CPT codes: 43226-43259) performed on-site at VA-CIHCS. Estimated 104-110 procedures per month, 1250 procedures per year. GI Colonoscopy Procedure: Colonoscopy procedures (CPT codes: 45378- 45398) performed on-site at VA-CIHCS. Estimated 61-66 procedures per month, 800 procedures per year. GI Sigmoidoscopy Procedure: Sigmoidoscopy procedures (CPT codes: 45330-45350) performed on-site at VA-CIHCS.
