A line item was added for Maintenance of Water Treatment and Dialysate Systems in accordance with the manufacturer s guidelines. See Performance Work Statement and Price/Cost Schedule for additional information. Offeror shall use the attached revised Price/Cost Schedule when submitting an offer. Offeror shall acknowledge this Amendment by either returning a copy of this Amendment or by annotating acknowledgement in their offer. The date/time for receipt of offers remains unchanged.
