Vendor Questions in bold, Responses in red 1. Does this site have a forklift onsite to unload the lift? To assist in installation of the lift? Yes. 2. Would you like hydraulic oil included in the sale of this lift? Yes. 3. Can you confirm that the concrete is the following quality or better: 4 inches minimum depth, with steel reinforcement, 3500 psi, cured for 28 days per local commercial practice: Yes, improvements to the pad are being performed now to bring it up to this standard. 4. Are we removing a lift to make room for the new lift? If so what kind of lift is there? No, the old lift has already been removed. 5. Confirm ceiling height is at least 164". No, the ceiling height is 12 11 (155 ). The ceiling height in the original statement of work was incorrect (153 ) and has been changed to reflect the correct ceiling height (155 ). 6. Can you please furnish pictures of the site? Yes, see below at the end of this document. 7. The lift specified (SPO12N7T5) is too tall for the space however, instead of the SPO12N7T5 the SPO12N7TL would fit with 10 to spare. Would that work? As long as the lift can function under the 155 ceiling height limitation and satisfies the other requirements established in the Statement of Work, we would find it acceptable. B.4 STATEMENT OF WORK FURNISH AND INSTALL 2-Post Automotive Lift Black Hills Health Care System - Fort Meade The Vendor shall provide all supervision, materials, equipment for furnishing and installation of a 2-Post Automotive Lift Rotary SPO12N7T5 (Brand name or equal) and accessories. 2-Post Automotive Lift s Salient Characteristics Rotary SPO12N7T5 (SPO12-TA) - 12,000 lbs Capacity, 2-Post Symmetric Lift w/ Front and Rear 3-STAGE Arms & Truck Adapters (Brand name or equal) Lift Capacity: 12,000 lbs. to be able to work on ¾ ton vehicles. Lift height: our ceiling is 12 11 (155 ) high so this would be the maximum height Minimum drive through width: 8 6 3/8 to be able to work on the bigger ¾ - 1-ton vehicles. Lifting adapters for lifting a variety of vehicles that have special lifting points such as: Ford Focus 2020, Ford F250, Ford Ranger 2008, Hyundai Elantra, Chevy Equinox, GMC Acadia, Dodge Ram 2500 and the Dodge Promaster. Works with 2 HP electric motor with a 220v minimum Accessories Maintenance Manuals (CD, PDF or Word) User Manuals In Service Training- The vendor must be able to provide in-service training for use and everyday maintenance of the equipment to help reduce operating expenses and unexpected equipment failures. Place of Performance The selected vendor shall provide equipment to: Department of Veterans Affairs Black Hills Health Care System Building T171 113 Comanche Road Fort Meade, SD 57741 Meade County Period of Performance The contractor shall complete the work required under this SOW between August 20th - September 10th, 2023, from the date of award unless otherwise directed by the Contracting Officer (CO). If the contractor proposes an earlier completion date, and the Government accepts the contractor s proposal, the contractor s proposed completion date shall prevail. Work at the Government site shall not take place on Federal holidays or weekends unless directed by the CO.
