Inability to provide the required service will be annotated through Contract Discrepancy Reports and identified in the Government Contractor Performance Assessment Reporting Service (CPARS) which may affect future contract opportunities. Please ensure adequate and qualified candidates are available before submitting an offer. This a combined synopsis/solicitation for commercial items prepared in accordance with the format in the Federal Acquisition Regulation (FAR) Subpart 12.6, as supplemented with additional information included in this notice. This is a solicitation conducted using FAR Part 13, Simplified Acquisition Procedures. This announcement constitutes the only solicitation, and a Request for Quote (RFQ) is being requested. Solicitation Number: 36C26323Q0065 The solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular (FAC) 2022-08 dated 10/28/2022. This solicitation is set-aside for Service-Disabled Veteran Owned Small Business (SDVOSB) entities. At the time of submission of offer and prior to award, the offeror shall be listed as verified in VIP database at https://www.vip.vetbiz.va.gov and verified and active as a Small Business in the System for Award Management (SAM) reflecting NAICS code 561320 Temporary Help Services with a small business size standard of $30.0 Million. The PSC Code is Q521, Medical - Pulmonary. The VA anticipates the award of one (1) Firm Fixed Price (FFP) contract for Board Eligible/Board Certified Pulmonary Physician Service, 1.0 FTE, On-site Service to support the Sioux Falls VA Health Care System located at 2501 W 22nd Street, Sioux Falls SD 57105.
