This is a solicitation conducted using FAR Part 13, Simplified Acquisition Procedures. This announcement constitutes the only solicitation, and a Request for Quote (RFQ) is being requested. Solicitation Number: 36C26323Q0017 The solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular (FAC) 2022-06 dated 5/26/2022. This solicitation is set-aside for Service-Disabled Veteran Owned Small Business (SDVOSB) entities. At the time of submission of offer and prior to award, the offeror shall be listed as verified in VIP database at https://www.vip.vetbiz.va.gov and verified and active as a Small Business in the System for Award Management (SAM) reflecting NAICS code 621111 ) Offices of Physicians (except Mental Health Specialists) with a small business size standard of $12.0 Million. The PSC Code is Q509, Medical-Internal Medicine. The VA anticipates the award of a Firm Fixed Price (FFP) contract for medical services necessary to perform onsite outpatient Primary Care and Urgent Care clinical services. The contractor shall provide these services at the Department of Veterans Affairs Montevideo Community Based Outpatient Clinic in Montevideo, Minnesota. See Performance Work Statement for specific requirements and qualifications. Period of Performance is from Estimated December 1, 2022, to November 30, 2023. The Department of Labor (DOL), Service Contract Act (SCA), Wage Determination (WD) Labor Standards applies to services provided under the contract and is provided as an attachment to this announcement. The WD specifies the minimum wages and fringe benefits for workers performing services on or in connection with the contract. Solicitation Questions.
