This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in the Federal Acquisition Regulation (FAR) FAR 13 and FAR Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation, and a Request for Proposal (RFP) is being requested. Solicitation Number: 36C26322R0154 The solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular (FAC) 2022-06. The VA anticipates the award of one Firm Fixed-Price (FFP) Contract for all resources and labor necessary to provide Health Care for Homeless Veterans (HCHV) Services to support the Department of Veterans Affairs, Sioux Falls VA Health Care System. The contract contains a base period of performance and includes four (4) option periods to be exercised at the discretion of the Government. Review the solicitation document, Statement of Work, Price/Cost Schedule, and Evaluation Factors for specific requirements and submittals. This procurement is unrestricted. At the time of submission of offer, offerors shall have an active System for Award Management (SAM) registration reflecting a NAICS Code 624229, Other Community Housing. The Department of Labor, Service Contract Labor Standards may apply to this procurement. Offerors shall identify whether the Service Contract Labor standards apply. Refer to FAR Provision 52.212-2 within the attached solicitation document for information regarding the evaluation of offers. Offer Due Date: Quotes are due to the Contracting Office by September 5, 2022, at 11:00 AM CST. Quotes shall be emailed to anna. leitzke@va.gov prior to the solicitation closing date and time and include Solicitation Number 36C26322R0154 in the Subject Line of the email. It is the responsibility of the offeror to ensure their quote is received in full by the Contracting Office before the solicitation closing date and time.
