If you believe your firm is able to provide the items requested below, you are encouraged to respond by the due date with all of the required information indicated below in order to be considered. When submitting a response to this RFI, offerors must include the following: Indicate socio-economic status Include picture of offered items and support literature, brochure etc. Indicate any existing Government contract where item(s) may be purchased (i.e. FSS/GSA contract etc.) Point of contact, DUNS number, complete mailing address, telephone number & email address Proof of authorized distributor status for ALL items submitted for consideration If suggesting an Or Equal product(s), include information that demonstrates that product(s) meets/exceeds salient characteristics Manufacturer Information of the product that your company/entity can provide to include: Manufacturer Name, Address, and DUNS Number. (c) This solicitation is issued for the purpose of: conducting market research to identify potential sources for a potential procurement of BRAND NAME OR EQUAL Window treatments as indicated below at the Minneapolis VA Health Care System NAICS: 334514 Totalizing Fluid Meter and counting Device Manufacturing Size Standard: 750 employees Salient Qualities: Chem-DAQ Steri-Trac 2 Monitors with Acrylic Stand air quality monitoring system. Must be able to detect Peracetic Acid, Hydrogen Peroxide. Must have data acquisition system to include hardware and software. Data acquisition system must have it s own power back up in case of electrical outage. Needs to be able to be monitored from a central location outside of the working area. Must be able to provide in house training to 50+ staff working 24/7. Vendor will be responsible for removal and disposal of current system. Vendor needs to be able to install new system without disrupting workflow. Vendor must be able to replace any and all hardware, wiring, and mounts without causing dirt and/or debris to become airborne. This system will be in a clean environment and if vendor will cause dirt and/or debris it will require a shut down of entire department and stop patient care until department has been certified to resume work.
