The Fargo VA Healthcare System is finishing a new construction project that will enable our Physical Therapy and Occupational Therapy staff to provide additional care. Part of that expanded patient care is in the use of mat tables in the treatment of back and other mobility limiting injuries. The purchase of a bariatric mat table will enable us to treat as many types of patients as needed. THIS IS A BRAND NAME OR EQUAL (BNOE) REQUIREMENT This statement of work is for the purchase of a bariatric mat table for use in the new PT/OT area. The contractor shall install new equipment only. No refurbished, returned or used equipment is permitted. REQUIREMENTS A. Bariatric Mat Table Motorized EQUAL TO OR SIMILAR TO TRI W-G Bariatric Motorized Mat Table B. The Mat Table must have the following characteristics in order of importance: Motorized height adjustable from 18”- 38” Motorized raised back At least 1000 lbs weight capacity Have a hand-held controller for motorized functions Casters with lockable brakes for easy movement and patient safety Available with a Sit to Stand accessory Steel frame with rounded edges for durability and patient safety All Equipment and accessories must be present upon delivery. All items must be shipped F.O.B. Destination. SHIPPING SHALL BE INCLUDED IN THE PRODUCT PRICE AND SHALL NOT BE BILLED SEPARATELY. Any additional material to include the standard warranty information, training and cleaning materials and proper care for upkeep are required upon delivery.
