Statement of Work Chemo Hood Demo and Hood Disposal Vendor to provide service on the decontaminating of Two Nuaire chemo hood vents and dismantling them for transport and disposal from the VA Medical Center, 4801 Veterans Drive, St. Cloud, MN, 56303. Vendor Conditions: Proposal is based on the following assumptions and site conditions. Any work which falls outside of the assumptions will constitute work beyond the intended scope and be completed upon mutually satisfactory terms. For the removal of model numbers NU-NR797-400, and NU-PR797-400, cleaning and demoing the Two hoods units only. Vendor will supply containers for transporting generated waste. Based on a flat day rate including travel. Includes waste transportation and disposal pending an approved waste profile. Units are removed from service and not attached to anything. Area available for Decon pool, 10-foot X 10 foot. Access to electrical and water utilities within 100 feet of the work area. Payment will be made upon receipt and approval of all documentation. Any deviations in work schedule will be approved by the Contracting Officer.
