The purpose of this announcement is to gain knowledge of current market conditions relative to NAICS 624229 Other Community Housing Services with a size standard of $16.5 million. The Department of Veterans Affairs (DVA), Network Contracting Office 23 (NCO 23), on behalf of St. Cloud VA Health Care System (SCVAHCS), St. Cloud, MN is seeking to identify any vendors capable of providing contractual services of specialized specific diagnostic testing in accordance with the specifications contained herein to beneficiaries of the DVA and SCVAHCS. The period of performance will be one (1) twelve-month base period, and three (3) twelve-month option years; estimated start date is November 1, 2020 through October 31, 2021. The St. Cloud Veterans Affairs Medical Center is looking for responses from contractors regarding their capability to provide the following mentioned services. Services to be provided include: 1. Contingent upon VA veteran referral and contractor acceptance, the contractor will accept homeless, including mentally ill and or chemically dependent veterans for Transitional Living only after they are specifically authorized by the St. Cloud Veterans Affairs Medical Center, Health Care for Homeless Veterans (HCHV) Program, at St. Cloud MN, telephone (320)252-1670, ext. 7190 Fax: (320)255-6423. It is understood that the type of Veteran to be cared for will not normally require contractor care and/or services over and above the level of care provided to non-Veteran guests. 2. The contractor will furnish each Veteran authorized care under this contract with the following services: a. A supervised shelter/residential environment with 24-hour staffing. b. Continual access to all buildings, facilities, and resources necessary for Veterans residing in the facility. c. Assigned, secure, furnished room space for veterans and their belongings, including an acceptable place to sleep, attend to personal hygiene and a properly equipped dining area to consume food that is safe, sanitary, and complies with all applicable codes and regulations. d. All assigned space must be structurally sound and have natural or mechanical ventilation, properly operating environmental control, adequate natural or artificial devices while assuring safety from fire. e. Laundry is provided on-site (or an alternative is present and agreed upon). 3. To address the nutritional and incidental needs of Veterans authorized care, the contractor will: a. Maintain regulatory requirements pertaining to the storage, preparation, serving, and disposal of food items. b. Normally, there will be three eating opportunities daily. Breakfast can consist of cereals, breakfast rolls, assorted fruits and drinks. Lunch and Dinner meals will be prepared, hot and nutritious. On holidays/weekends, contractor has the option of preparing a "Brunch" meal in place of Breakfast and Lunch. c. On admission, provide each Veteran a starter kit, including bedding, towels and toilet articles. d. Collaborate with HCHV staff to assure VA residents with special dietary needs, i.e., diabetes, hypertension, etc., are seen in the HCHV clinic and referred to a Registered VA dietician for consultation and additional direction in selecting and preparing their meals. e. Collaborate with HCHV staff to provide transportation services, as needed, to support veterans seeking employment, obtaining identification or routine medical/psychiatric care, etc. 4. Through collaboration with HCHV staff, the contractor will assist in coordinating those rehabilitative services deemed appropriate but not to be limited to: a. Individualized, social/vocational rehabilitative case-management. b. Community coordination to facilitate veteran's social and vocational transition to stabile and/or independent living. c. Coordinated employment assistance with VA/State Vocational Rehabilitation, Job Services, and local employers to help Veterans secure employment or access technical or academic educational resources. d. Peer support, group counseling and activities, including physical activities as determined appropriate by HCHV caseworker, contractor staff, and veteran preference. e. Facilitate attendance at community-based recovery support programs to reinforce a sober lifestyle and social reintegration with a long-range plan of continued attendance after completion of the Transitional Living experience. f. Opportunities for learning, testing, and internalizing knowledge of illness/recovery process, and upgrading social skills/improving social relationships. g. Provide a supportive environment that is conducive to an alcohol/drug abuse-free lifestyle. h. Veterans will self-administer prescribed medications. i. Supportive social services in collaboration between contractor and HCHV caseworker. 5. The contractor will make available to the VA documentary evidence deemed necessary by the VA to conduct utilization review audits for the mandated national evaluation study as required by Section 2 of Public Law 100-6 as repealed by section 115 of Public Law 100-322; and Section 201 (b) of Public Law 101-366, as amended by Public Law 102-83; to verify quality patient care for veterans; to assure confidentiality of patient record information; and to determine the completeness and accuracy of financial records. This includes, but is not limited to: a. An individual record which complies with requirements of confidentiality to be maintained by the contractor on each veteran under contract placement. b. A documented admission summary reflecting the reason for referral, identifying information including his/her admission date, social/cultural vocational status, family or significant other, goals for change, anticipated length of stay, and a list of prescribed medications. c. Periodic reports to the VA, as required, including billing for contract costs which are to be submitted to the VA Fiscal Service monthly through the HCHV Coordinator for auditing, fiscal accountability statements and data for the National Evaluation Study. 6. The contractor will employ sufficient professional staff to carry out the policies, responsibilities, and programs of the facility. In a residential living facility, there must be a fulltime administrative staff member or designee of equivalent professional capability on duty, on the premises or residing in facility and available for emergencies 24 hours a day, 7 days a week. 7. Unless specifically excluded in this contract, the lease rate of $60/day will include the services listed in this document for five veterans in single-room occupancy transitional housing, with a provision of increasing to additional veterans with bilateral agreement in the future. These services will also include those sundry items/services normally provided other residents by the facility without extra charge. 8. The contract facility must have a current occupancy permit or license required by the authority having jurisdiction. 9. The contractor will comply with the VA Patient's Bill of Rights as set forth in Section 17.34a, Title 38 and Code of federal Regulations.
