This is a sources sought announcement only. This is not a solicitation announcement. The purpose of this announcement is to gain knowledge of current market conditions relative to NAICS 621111 Offices of Physicians, Ophthalmology Physician Service with a size standard $12 million. The Department of Veterans Affairs (DVA), Network Contracting Office 23 (NCO 23), on behalf of Sioux Falls VA Health Care System (SFVAHCS), Sioux Falls, SD is seeking to identify any vendors capable of providing contractual services of onsite ophthalmology physician services to eligible veteran beneficiaries of the SFVAHCS. The period of performance will be one (1) twelve-month base period, and four (4) twelve-month option periods. Background: Pursuant to the Veterans First Contracting Program, the VA is trying to ascertain if there are qualified Service Disabled Veteran Owned Businesses (SDVOSB) or Veteran Owned Small Businesses (VOSB) with the capability of providing the below services (Respondents are not limited to SDVOSB/VOSB concerns; the Government seeks to identify ALL potentially interested sources): 1. SERVICES PROVIDED The Sioux Falls VA Health Care System, hereinafter referred to as SFVAHCS intends to secure ophthalmology physician services for the SFVAHCS located at 2501 W 22nd Street, Sioux Falls, SD 57105. The contractor shall propose a minimum of two (2) key personnel to be available for scheduling to meet the estimated requirement of 0.5 FTE. One FTE is defined by VA as a person working 40 per week or a minimum of 80 hours every two (2) weeks and does not include holidays. The anticipated contract will be an Indefinite Delivery Indefinite Quantity contract, which actual dates and quantities ordered are not guaranteed. All services provided shall be in accordance with the costs, terms and conditions of the contract. 2. CONTRACT TYPE AND TERM The VA anticipates the contract will be an Indefinite Delivery Indefinite Quantity contract. Contract use will be limited to the Sioux Falls VA Medical Center only; no other VAMC may use this contract. The base twelve-month period of the contract shall begin January 1, 2021 with options to extend the contract for additional twelve-month periods, at the discretion of the VA, for up to four (4) additional periods. Total contract length of five years. 3. SCOPE OF SERVICES The SFVAHCS, requires the services of a contractor to provide ophthalmology physician services for the SFVAHCS. The contractor s physician(s) care shall cover the range of ophthalmology services as would be provided in a state-of-the-art civilian medical treatment facility and the standard of care shall be of a quality, meeting or exceeding, currently recognized national standards as established by the American Academy of Ophthalmology (AAO). Responses to this sources sought notice must include the following information. 1. Socio-economic status of business (i.e. SDVOSB/VOSB, Small Business, Large etc.) 3. Are you, or have you provided Ophthalmology Physician Services. If so, please provide contract number(s) for the previous three (3) years. 4. Will your company enter into a joint venture or other relationship in order to provide this service?
