This is a Request for Quotation (RFQ) for a firm-fixed-price contract to supply a specific brand-name dental equipment system, the DENTALEZ Titan 3 Straight Nose Cone System, to the VA Phoenix Healthcare System in Phoenix, AZ. The contract is set aside for Service-Disabled Veteran-Owned Small Businesses (SDVOSB). Delivery of the items is required within 30 days after receipt of order to the specified VA warehouse in Phoenix.
