Participants will meet at VA Greater Los Angeles Healthcare System, 11301 Wilshire Blvd, Los Angeles, CA 90073-1003. Plans for the Community Based Outpatient Clinics (CBOCs) will be available. Participation in the site survey is not mandatory. Please email israel.garcia2@va.gov no later than Monday September 9, 2024, to identify participants. Submit any questions regarding this survey via email to israel.garcia2@va.gov no later than 12:00PM PST Thursday, September 12, 2024. No questions will be answered after this date/time unless determined to be in the best interest of the Government as determined by the Contracting Officer. Telephonic questions will not be accepted or returned. RFI responses will be responded to as necessary in amendment format which will be posted to contract. Extend the solicitation due date to Wednesday September 18, 2024, by 12:00PM PST. Submit written offers with spec-sheets of proposed items in accordance with the following instructions, as well as Addendum to FAR 52.212-1, Instruction to Offerors. Oral offers will not be accepted. Complete and submit enclosure 1 via email no later than, Wednesday September 18, 2024, by 12:00PM PST, to israel.garcia2@va.gov. There are no other changes to the terms and conditions of this Solicitation. All other terms and conditions remain in full force and effect.
