(i) This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Federal Acquisition Regulation (FAR) Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; offers are being requested and a written solicitation will not be issued. (ii) The solicitation number 36C26222Q1601 is issued as a Request for Quotation (RFQ). (iii) The provisions and clauses in this document are in effect through the Federal Acquisition Circular 2022-07 Effective August 10, 2022. (iv) This solicitation is set aside for small businesses currently registered at SAM.gov. The associated North American Industrial Classification System (NAICS) code for this procurement is 511130, with a small business size standard of 1,000 Employees. The FSC/PSC is 7610. (vi) Delivery Requirements: Place of Delivery VA Greater Los Angeles Healthcare System 11301 Wilshire Blvd Los Angeles, CA 90073 Period of Performance/Delivery Timeframe: All deliveries must be completed by 30 days from the date of the award or Offeror will provide estimated delivery date, specified in calendar days. (vii) All offers for this solicitation must be received electronically through email to Sam Choo at sam.choo@va.gov no later than Tuesday, August 30, 2022, 10:00 AM PT. Any questions regarding this solicitation must be received electronically through email no later than Friday, August 26, 2022, 10:00 AM PT. (viii) Offerors shall submit the following via email: Offeror shall complete 30a, 30b, and 30c of Standard Form 1449 and submit as part of the offer. Offerors shall fill out all unit price, total item cost, and net item cost conforming to requirements stated by the salient characteristics in Item Information (Section B.3) or provide a separate document with a complete list of items. Offerors shall fill out estimated delivery date in the Delivery Schedule (Section B.4) or provide a separate document with complete list of items. Offerors shall complete Offeror Representations and Certifications (Section D.5) and submit as part of the offer.
