Currently utilizing very outdated technology with very little to no parts availability, and a change in VA Directive regarding water testing and legionella as pushed to remove the water evaporative cooling tower currently serving the freezers and refrigerators. Update and repair the walk-in unit boxes to prevent frost buildup on adjacent walls and to increase insulation effectiveness. There are several ancillary HVAC systems that require repair, replacement or updating to include chilled water pumps in B148, 151, 162, Energy Recovery systems in B161 and B158 and a main line steam leak in B107. Update and install flow switches in building 162. Conduct a full HVAC assessment in Building 107 as it has multiple HVAC systems and controls in various states of disrepair. Update Building Automation Systems (BAS ) and HVAC control devices in Buildings 108, 158, 162, 164

Draft Scope of Work Various Building HVAC Repairs Date: 06/29/2022 Northern Arizona VA Healthcare System 500 Highway 89 N Prescott, AZ 86313 CONTENTS: Background Project Objectives Statement of Work Requirements Special Technical Requirements Attachments BACKGROUND The outdated Heating, Ventilation, Air Conditioning (HVAC) Systems for the Kitchen Building 107 walk in refrigerators and freezes on basement floor and first floor are very old, utilizing very outdated technology with very little to no parts availability and a change in VA Directive regarding water testing and legionella as pushed to remove the water evaporative cooling tower currently serving the freezers and refrigerators. PROJECT OBJECTIVES: Replace the cooling equipment to the 6 existing walk in freezers and refrigerators in Building 108-Kitchen STATEMENT OF WORK/REQUIREMENTS: Contractor to provide all materials, equipment, parts, labor and supervision to complete each one of the CLIN s as noted below. CLIN 0001: Building 148 Pump Repair. Contractor to provide all materials, equipment, parts, labor and supervision to rebuild each of the heating hot water pumps. CLIN 0002: Building 151 Pump Repair. Remove one pump at a time, total (2) as shown in the picture below and rebuild, including bearings, seals, gaskets and pint. Install new motors Upon reinstallation, install new coupling inserts and alight to within 0.003 horizontally and vertically. Install new piping, gauge and ball vales to measure the pressure across the pump (Grey piping on the pump shown below) CLIN 0003: Building 161 Energy Recovery Wheel. Replace the energy recovery wheel in the energy recovery unit in the attic of building 161. This will require the taking apart of the unit, removing the damaged energy wheel and install a new OEM energy wheel with new motor and belt. The Total Enthalpy Wheel details will need to be field verified, but the basis of design was a Greenheck ERV-522S-15-A. CLIN 0004: Building 162 Chilled Water Valve. Replace the Honeywell MN7505A2001 three-way chilled water valve as shown in picture below). Install new gaskets, repair insulation necessary to make repair and reconnect and test operation through the building automation system. CLIN 0005: Building 158 ERV Controls The Energy Recovery Ventilator on the roof of Building 158 was found to have control wires run from the unit to the building automation controls panel in the mechanical room, but never terminated. Contractor to terminate the control wires at the ERV and at the new Distech Controller in the B158 Mechanical room. Items to manage and monitor in the controls to be the ERV and Wheel start/stop/status and graphics, alarms and trending in the Distech Controls. CLIN 0006: Building 162 Flow Switches The heating hot water and domestic hot water flow switches in the basement of building 162 are fault. Contractor to replace the four (x4) flow switches, terminate to existing controls, test and validate to ensure proper operation. CLIN 0007: Building 107 HVAC Assessment. Contactor shall conduct an assessment of the existing mechanical equipment and HVAC controls to determine the life span and current operations of the equipment and a list of equipment not operating properly. The assessment will also detail out information about each of the mechanical equipment, piping, controls, valves, actuators, filter sizes/conditions, locations of each unit. Areas included are as follows. Many of the area are in patient care areas and plan for four (4) weekend days as well as the remaining to be completed during normal business days. B107-1B- 17 Fan Coil Units B107-2A- 11 Fan Coil Units B107-3B- 24 Fan Coil Units B107-4A- 2 Rooftop Units (RTU) and 13 Variable Air Volume (VAV) Units B017-4B- 1 Rooftop Unit (RTU) and 8 Variable Air Volume (VAV) Units B108- 3 Rooftop Units (RTU), 2 Makeup Air Units and 3 Package Rooftop Units CLIN 0008: Distech Niagara System Upgrade. Buildings 108, 158, 161, 162 and 164x2 are local JACE 8000 Distech Niagara building automation controls. The better manage and provide a campus management type system, there is a need to add a server to support the buildings noted above. As such, the contractor shall provide all labor, materials and supervision to complete the following Migration to a server campus style system, where server, to be located in B28. The new Dell Power Edge Technologies rack mounted server, with CORE-100 device license for future devices New laptops (x2) with Niagara Distech Software installed on both hosts Laptop shall be commercial rugged standard with supporting 64-bit hardware (as limited by the direct-digital control system software) and software enterprise server. Windows 10 40 hours of hybrid training (On-Site/Remote) Remote to be in a minimum of one (1) hour and onsite training a minimum of four (4) hours Distech NE-Net Supervisor-100 For 100 Niagara Network connections CLIN 0009: Building 107 Steam Leak Repair The main steam valve for the main hospital, building 107 is leaking and as such, the contractor will need to: Remove the existing steam gate valve, install new gate valve (Type 101-ASTM A126 Steel body, rated 1035 kPa (150 psig) at 260 degrees C (500 degrees F), 11-1/2 to 13 percent chromium stainless steel flexible wedge and hard faced (stellite) or nickel copper alloy seats, 1035 kPa (150 psig) flanged ends, OS&Y, rising stem, bolted bonnet. Factory installed globe valved bypass on all steam valves greater than 75 mm (3 inches). Drill and tap bosses for connection of drains where shown. Install new gaskets. Flanged Joints: Select gasket material, size, type, and thickness for service application. Install gasket concentrically positioned. Use suitable lubricants on bolt threads. Gaskets and bolting shall be applied in accordance with the recommendations of the gasket manufacturer and bolting standards of ASME B31.1. Strains shall be evenly applied without overstress of bolts. Gaskets shall cover entire area of mating faces of flanges. Reinsulating piping disturbed by the work (Match existing) Installation of the thermal insulation removable blanket around the new valve. Work to be completed on a weekend to minimize the disruption to the patients and the steam shall be operational on the same day as the when the steam is turned off. Time requirement to complete the project: as these buildings serve employee work areas, contractor shall make any attempt to complete the project within a minimum possible time and contractor shall ensure they have all materials to complete the project without any delay VHA Supplemental Contract Requirements for Combatting COVID-19 Contractor employees who work in or travel to VHA locations must comply with the following: Documentation requirements: If fully vaccinated, contractors shall show proof of vaccination. NOTE: Acceptable proof of vaccination includes a signed record of immunization from a health care provider or pharmacy, a copy of the COVID-19 Vaccination Record Card (CDC Form MLS-319813_r, published on September 3, 2020), or a copy of medical records documenting the vaccination. If unvaccinated, contractors shall show negative COVID-19 test results dated within three calendar days prior to desired entry date. Test must be approved by the Food and Drug Administration (FDA) for emergency use or full approval. This includes tests available by a doctor s order or an FDA approved over-the-counter test that includes an affiliated telehealth service. Documentation cited in this section shall be digitally or physically maintained on each contractor employee while in a VA facility and is subject to inspection prior to entry to VA facilities and after entry for spot inspections by Contracting Officer Representatives (CORs) or other hospital personnel. Documentation will not be collected by the VA; contractors shall, at all times, adhere to and ensure compliance with federal laws designed to protect contractor employee health information and personally identifiable information. Contractor employees are subject to daily screening for COVID-19 and may be denied entry to VA facilities if they fail to pass screening protocols. As part of the screening process contractors may be asked screening questions found on the COVID-19 Screening Tool. Check regularly for updates. Contractor employees who work away from VA locations, but who will have direct contact with VA patients shall self-screen utilizing the COVID-19 Screening Tool, in advance, each day that they will have direct patient contact and in accordance with their person or persons who coordinate COVID-19 workplace safety efforts at covered contractor workplaces. Contractors shall, at all times, adhere to and ensure compliance with federal laws designed to protect contractor employee health information and personally identifiable information. Contractor must immediately notify their COR or Contracting Officer if contract performance is jeopardized due to contractor employees being denied entry into VA Facilities.
