The Phoenix VA Health Care System (PVAHCS) located at 650 E. Indian School Rd., Phoenix, Arizona 85012 is seeking a qualified/OEM certified Medtronic PB980 intensive care ventilators maintenance and repair contractor. The contractor is to supply labor, materials, OEM parts/software and equipment to provide the following: Required Services The Contractor shall provide preventive maintenance. The Contractor shall provide repair services. The Contractor shall respond to non-emergent calls with (4) hours of the initial call. The contractor shall respond to emergent calls within (1) hour of the initial call. The Contractor shall be on site within (48) hours of dispatch. The Contractor shall be responsible for all labor. The Contractor shall be responsible for all travel. The Contractor shall be responsible for all parts Including batteries Excluding consumables such as patient circuits and filters Service technicians shall be certified by Medtronic (formally Covidien, formally Puritan Bennett) to perform any services on the enclosed specified equipment. Contractor shall provide documentation/copy of certification with proposal submittals and upon award.
