Page 1 of 1 This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in FAR Subpart 12.6 -- Streamlined Procedures for Evaluation and Solicitation for Commercial Items, in conjunction with FAR -- Part 13 Simplified Acquisition Procedures, specifically FAR Subpart 13.5 Simplified Procedures for Certain Commercial Items; and supplemented with additional information included in this notice. This announcement constitutes the only solicitation; offers are being requested and a written solicitation is attached to this announcement. Solicitation # 36C26221R0095 is issued as a Request for Proposal (RFP) and constitutes the only solicitation. Description of Services: The Contractor shall provide respirator fit testing services as described in the Statement of Work of the attached solicitation. The contractor shall perform fit testing and training using OSHA-approved testing procedures and media and shall include supplies and labor to perform scheduled services Monday through Friday, excluding national holidays as described in the SOW for the VA Loma Linda Healthcare System (VALLHS), the VA Long Beach Healthcare System (VALBHS), and the VA Greater Los Angeles Healthcare System (VAGLAHS), and the VA San Diego Healthcare System (VASDHS). The line items and estimated quantities for each facility are identified in attachment D.5 of the attached solicitation.
