This is a sources sought/RFI only. The purpose of this sources sought/RFI is to gain knowledge of potential qualified sources such as Service-Disabled Veteran-Owned Small Businesses (SDVOSBs), Veteran-Owned Small Businesses (VOSBs), Hub-zone firms, general small business firms and their size classification relative to NAICS 621498 (size standard $20.5 Million). Responses to these sources sought will be used by the Government to make appropriate acquisition decisions. After review of the responses to this sources sought, a solicitation announcement may be published. Responses to this sources sought synopsis are not considered adequate responses for a solicitation announcement. The Network 22 Long Beach Contracting Office (NCO 22L) is seeking small business sources for establishing a Community Based Outpatient Clinic (CBOC) in Gardena, California. The Department of Veterans Affairs Health Care System Long Beach (VALBHS) has a requirement for the provision of Primary Care Services, Mental Health Services, Specialty Care, and Telehealth Services for eligible Veteran beneficiaries who desire to receive patient care in Gardena, California. The contractor facility must provide services exclusively to Veteran patients only. Interested firms having the skills and capabilities necessary to perform the stated requirements are invited to provide information to contribute to the market survey/sources sought notice. Responses to this notice must provide the following information: General information of the company name, address, phone number, fax number, e-mail address(s), and company website. How long your company has been in business What services does your company primarily provide and for how long. If your company have experience with the Department of Veterans Affairs capitated CBOC program What relevant Government and commercial experience does your company have related to the establishment of CBOCs. Describe how you intend to provide the Capitated CBOC services for our Veterans Describe your company ownership composition listing the names of individuals that control and have authority to enter into a contract with the Government and their percentage of ownership of your company If your company be establishing a joint venture with another company If your company meets the standards of the Joint Commission (JC) If your business is small relative to NAICS 621489, size standard $20.5 million If small, does your firm qualify as small, emerging business, or small disadvantaged business If disadvantaged, specify under which disadvantaged group and if your firm is certified under Section 8(a) of the Small Business Act? If your firm is a certified HubZone firm If your firm is a woman-owned or operated business If your firm is a certified Service-Disabled Veteran-Owned Small or Veteran owned small business Provide your Dun and Bradstreet number. The information identified above is intended to be descriptive, not restrictive and to indicate the quality of the services that will be satisfactory. It is the responsibility of the interested source to demonstrate to the Government that the interested parties can provide the services that fulfill the required specifications.
