This is a combined synopsis/solicitation for commercial services/items prepared in accordance with the format in FAR Subpart 12.6 in conjunction with FAR Part 13, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; quotes are being requested and a separate written solicitation will not be issued. 1.a. Project Title: Imaging Service Off-Site Film/Records Storage 1.b. Description: VA Loma Linda Healthcare System is seeking a contractor to provide storage of Radiology and Nuclear Medicine films in a NARA certified facility. 1.c. Project Location: Contractor s facility 1.d. Solicitation Type - Number: Request for Quotes (RFQ) - 36C26220Q0167. 1.e. Set Aside: This solicitation is not set aside. 1.f. Applicable NAICS code: 493190, Other Warehousing and Storage 1.g. Small Business Size Standard: $27.5 Million 1.h. Type of Contract: Firm Fixed Price 1.i. Period of Performance: Base Year with Four (4) Option Years 1.j. Wage Determination applicable to this project: SCA WD 2015-5630 revision 12 2. Statement of Work 2.1. Scope. The contractor will be responsible for the safe storage of archived Radiology and Nuclear Medicine films while protecting the integrity of patient information and keeping the film and records from physical deterioration in a NARA compliant facility. 2.2. Specific Tasks. 2.2.a. Store archived Radiology and Nuclear Medicine films. 2.2.b. Purge any, and all outdated X-ray films as required by the NARA specified schedule for destruction and disposal of Radiology and Nuclear Medicine films. 2.2.c. Maintain the storage of Radiology and Nuclear Medicine films as specified by NARA and VA. 2.2.d. Must be a NARA compliant facility and adhere to National Institute of Standards and Technology Guidelines for Media Sanitation (NIST SP 800-88) as required by the HITECH Act of 2009 2.3. Performance Monitoring. 2.3.a. Contractor must maintain NARA certification for the facility. 2.3.b. Facility POC will monitor storage of records. 2.4. Security Requirements. Contractor will not have access to VA Systems or VA records. 2.5. Period of Performance. January 1, 2020 to December 31, 2020 as base period with four option years 3. The solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 2019-02, dated 5/6/2019.
