Pre-Solicitation Notice of Intent to Sole Source Department of Veterans Affairs (VA), Network Contracting Office (NCO) 21 intends to award a sole source, Firm Fixed Price, Indefinite Delivery Indefinite Quantity (FFP-IDIQ) contract to Hale Nani Nursing and Rehab Center, LLC, (UEI: VF7YYCCHLQY9). The Contractor is currently providing community nursing home (CNH) services to 15 Veterans at the facility and a replacement contract is being established to provided continued care for the Veterans at the CNH. All responsible sources that can provide community nursing home services within Honolulu, Hawaii may submit a capability statement for consideration on or before May 15, 2025, 12:00PM PST. After consideration, the Government will determine whether to proceed with the sole source acquisition. Please keep in mind that patient care and choice in the decision is the highest priority to the VA. As prescribed in VHA Notice 2020-32 and VHA Handbook 1143.2, for more than 35 years the CNH Program has maintained two cornerstones: some level of patient choice in choosing a nursing home close to the Veteran s home and family, and a unique approach to local oversight of CNHs. A Justification and Approval (J&A) will be executed by the Head of the Contracting Activity (HCA) for the contract and will be posted to sam.gov required by Federal Acquisition Regulation (FAR) 13.501(a)(1)(iii). Period of Performance: 11/01/2025 to 10/31/2027
