The Department of Veterans Affairs, Network Contracting Office 21, is conducting a market survey to identify potential sources to perform a firm fixed price contract as follows: DESCRIPTION OF REQUIREMENT (NOT COMPREHENSIVE) Contractor shall provide gene expression assay and Oncotype DX prostate cancer testing services for the Pathology and Laboratory Department at VA Northern California Healthcare System (VANCHCS), 10535 Hospital Way, Mather, CA 95655. CONTRACTOR REQUIREMENTS/QUALIFICATIONS: The contractor shall provide all personnel, equipment, tools, materials, supervision, other items, and non-personal services necessary to perform the following testing services: ConfirmMDx testing Prostate (CPT code 81551) SelectMDx testing Prostate (PLA code 0339U) Oncotype DS GPS testing Prostate (PLA code 0047U) The contractor shall provide specimen transportation kit boxes and all applicable forms necessary to perform the specific testing. The contractor shall send the test results to the VANCHCS. The contractor shall be licensed, certified, and be able to meet accreditation and certificates required by law. The contractor s laboratory must be accredited by the College of American Pathologist (CAP) or be able to meet the Joint Commission and Clinical Laboratory Improvement Amendments (CLIA) (Public law 100-578) certifications. Capabilities Statement and Documentation: Companies are invited to provide their capabilities, experience and knowledge in the provision of prostate cancer testing services.
