This is a notice of intent to sole source a contract under the authority of FAR 13.106-1(b)(2), only one source being available to meet the Government s need. The North American Industrial Classification System (NAICS) code for this procurement is 339113: Surgical Appliance and Supplies Manufacturing, with a small business size standard 800 Employees. The FSC/PSC is 6515: Medical and Surgical Instruments, Equipment, and Supplies. The Department of Veterans Affairs intends to solicit and award a sole source contract for the ACS Replenishment Kits to Vericor, LLC, 703 Western Ave, Holmen, WI 54636-9787. There are no other source of supply/service that will satisfy agency requirements to Vericor, LLC, 703 Western Ave, Holmen, WI 54636-9787 is the only vendor that can provide the requested Replenishment Kits. There is no other company that was identified that can meet this requirement. The Southern Nevada VA Health Care System is seeking to establish the following: Line Item Description Part Number Qty 1 Replenishment, Southern Nevada HCS, North Las Vegas, NV V11533 ACS Set A1, Expired Product thru April 2025 RF-R-V11533A1-2504 1 2 Replenishment, Southern Nevada HCS, North Las Vegas, NV V11533 ACS Set A2, Expired Product thru April 2025 RF-R-V11533A2-2504 1 3 Replenishment, Southern Nevada HCS, North Las Vegas, NV V11533 TT Sets G & Y, Expired Product thru December 2024 RF-R-V11533GY-2504 1 Any responsible source that feels it is capable in performing this requirement may submit a quote (monthly) for the above listed items and describe and/or submit documentation to support your capabilities. The response will be considered by the agency. If you are not the Original Equipment Manufacturer (OEM), you must submit an authorized distributor letter signed by the OEM. If you are not the OEM and a small business, you must indicate in your response how you will comply with limitations on subcontracting and/or the non-manufacturer rule. Responses should be submitted to Alex Hernandez at Alex.Hernandez@va.gov no later than August 5, 2024 at 11:00pm PT to be considered. If no responses are received, the Government will continue with its sole source procurement, and a sole source Request for Quote will be published to contract opportunities in accordance with FAR 5.102
