The purpose of the attached amendment 0004 is the following: A Provide Updated Statement of Work reflecting revision of a 90 day transition period. B. Provide responses to follow up questions. The purpose of amendment 0003 is the following: A. Extend the offer submission date to Monday, February 5th, 2024. B. Provide Updated Addendum 52.212-1, noting follow up questions are due by Thursday, January 18th, 2024, at 17:00 PST. C. Provide Updated Statement of Work. D. Provide Updated Contract Line Items. E. Provide responses to remaining questions. The purpose of amendment 0002 is the following: A. Provide answers and clarifications in attachment 01, 36C26124Q0072 0002; SNVHCS Las Vegas Home Oxygen Services and Supplies Questions, to clarifications requested and questions received. B.
