The contract shall be Cost per Test (CPT)/Cost per Reportable Result (CPRR), Clinical Laboratory Analyzers supporting VISN 21 in accordance with the Statement of Work. Period of Performance: Ordering Period 1: 4/1/2024 - 9/30/2024 Ordering Period 2: 10/1/2024 - 9/30/2025 Ordering Period 3: 10/1/2025 - 9/30/2026 Ordering Period 4: 10/1/2026 - 9/30/2027 Ordering Period 5: 10/1/2027 - 9/30/2028 Responses to the received questions regarding this RFQ are attached along with an amendment containing a revised Statement of Work. The NAICS is 334516 with Small Business size of 1,000 employees.
