VA Central California Health Care System PERFORMANCE WORK STATEMENT File Room Outsource, Conversion, VistA Indexing/Upload Services 1. SCOPE OF WORK: The VA Central California Health Care System (VACCHCS) in Fresno, California has a requirement for indexing of medical and administrative active record files into VistA Imaging, an electronic medical records system. These services include accurately attaching indexed images to the appropriate electronic health record. Accuracy is extremely important to maintaining the integrity of the medical record. The Contractor shall provide all resources necessary to accomplish the requirements described in this PWS. Specifically, the Contractor shall perform file room services, or scanning/importing and indexing of newly generated and backlogged clinical documentation for inclusion into the health record, including VACC documentation at specified VAMCs in a defined, accurate, timely, measurable, and fiscally responsible manner that protects the interests of VA. 1.1. PICK UP LOCATION: The contractor shall transport all records and provide storage in accordance with 36 CFR 1228, subpart K, certified by the National Archives and Records Administration (NARA). Records are to be picked up at the below location (if applicable). San Jose, CA 95131 1.2.1. MEDICAL RECORD MANAGEMENT SERVICE: All clinical documentation that shall be scanned and digitized shall be incorporated into VistA Imaging under the VA Health System Wide Area Network (WAN) for access by VA CCHCS employees. The functionality of scanning/importing and indexing clinical documentation replaces the action of loose filing newly created or received documents into the paper medical record. An interface to move/upload images and their associated indexing information to VistA Imaging is required when a commercial system shall be used. VACC documentation may be received in paper, secured messaging, facsimile, Third Party Administrator (TPA) Portal or via the Fee Basis Claim System (FBCS) Document Manager software. It is the intent of this contract to scan selected documents based on input from the facility s medical and HIM staff. Also, the intent of this contract is to scan/import and index clinical documentation that has yet to be incorporated into the Veterans health record. 1.3.1. MEDICAL RECORD STORAGE: Storage of original paper medical records until upload the EHR and quality assurance has been complete. Contractor shall provide a National Archival Record Administration (NARA) approved record storage facility for comprehensive box storage of all VA medical records. The contractor shall provide personnel who have understanding of the VA Records Control Schedule 10-1 Federal General Schedule of Records. 2. PERFORMANCE DETAILS: The Contractor shall make certain that appropriate physical care and security measures are taken to ensure that documents and health records and the VA information they contain are not damaged or compromised throughout the entire process from the point of receipt through storage. The Contractor shall be compliant with all VA established security policies to ensure VA data, whether captured electronically or on paper or other media are not compromised. The Contractor shall adhere to established VA physical security policy prior to receipt and while in possession of documents and health records offsite. 3. PERFORMANCE PERIOD: The period of performance shall be a base year and two 12 month option years with a start date of October 1, 2023, and ending on September 30, 2026. IAW FAR 8.405-3, the Government reserves the right to end this agreement at any time. 3.1. FEDERAL HOLIDAYS: Any work at the Government site shall not take place on Federal holidays or weekends unless directed by the Contracting Officer (CO). There are eleven (11) Federal holidays set by law (USC Title 5 Section 6103) that VA follows: Under current definitions, five are set by date: New Year's Day January 1 Juneteeth June 19 Independence Day July 4 Veterans Day November 11 Christmas Day December 25 If any of the above falls on a Saturday, then Friday shall be observed as a holiday. Similarly, if one falls on a Sunday, then Monday shall be observed as a holiday. The other six are set by a day of the week and month: Martin Luther King's Birthday Third Monday in January Washington's Birthday Third Monday in February Memorial Day Last Monday in May Labor Day First Monday in September Columbus Day/Indigenous People Day Second Monday in October Thanksgiving Fourth Thursday in November 4. PLACE OF PERFORMANCE: Contractor place of performance for scanning, importing, and indexing documents shall occur offsite from the VACCHCS. Performance may take place outside normal business hours (8:00 a.m. 4:30 p.m.) as identified by the Contractor. However, overtime for this work shall not be authorized. 5. TRAVEL: The VACCHCS does not anticipate any travel for the Contractor in the performance of the contract. 6. SCANNING AND INDEXING OF MEDICAL RECORDS: Scanning and/or Importing and Indexing of Records will be performed off site (at the contractor s location). All records shall be shipped to the contractor s location at the cost of the government if required. Therefore, when the scanning and/or importing and indexing of records have been completed, the contractor shall notify the COR that 100% Quality Assurance review of uploaded documents to EHR has been successfully completed and records are pending destruction approval. Documentation to be scanned and/or imported and indexed may include internal VHA sourced documents, externally sourced documents, including solicited VA Community Care (under VA auspices) or unsolicited documents provided by the Veteran or other non-VA provider for inclusion in the health record. 6.1.1 Contractor shall be required to scan documents with a minimum resolution of 300 x 300 dots per inch (dpi) as established by VistA Imaging specifications and ensure that images are correctly indexed, oriented, legible, and complete, including signature by the originating provider (VistA Imaging Clinical Capture User Manual). Scanned documents may be scanned in black/white, gray scale, true color, or 256 color (depending on the document. X-rays shall not be scanned, however, photographs (e.g., Polaroid s, etc.) shall be scanned. Facility records are a hybrid of typed and handwritten documents, whereas, the older records may contain more handwritten documents. 6.1.2. Contractor may use a vendor product, Secured Portal or secured messaging as the imaging capture system instead of directly scanning into VistA Imaging. If a vendor product, portal or secured messaging is used, the facility shall provide the guidance/policy on the requirements for using this program. The Contractor shall be required to import the documents into VistA Imaging with a minimum resolution of 300 x 300 dots per inch (dpi) as established by VistA Imaging specifications and ensure that images are correctly indexed, oriented, legible, and complete, including signature by the originating provider (VistA Imaging Clinical Capture User Manual). 6.1.3. Whether scanning or importing, the Contractor shall index the documents into VistA Imaging by appropriate index term (e.g., Fee, application, correspondence, advance directive, medical record, flow sheet,), and ensure the indexing values are assigned properly. The local facility shall provide the Contractor with guidance to identify the indexing values for all document types. At a minimum there are six (6) data points required for indexing: date of document, document type, specialty, event/procedure, origin, and a free text description. Due to the variety of documents the Contractor shall scan and/or import, documents shall need to be reviewed in order to determine the correct indexing values. To ensure accurate indexing, the local facility COR shall work with the Contractor to identify document index terms and shall update the list accordingly as new document index terms become available. Documents shall be imaged as an exact replica of the original document with quality control processes in place with assurance that the imaged document is stored, accessible, and retained according to the 75 year retention requirement. 6.1.4. The Contractor shall make every attempt to identify unclear information and, as necessary, request assistance/clarification from the facility point of contact specified in the contract. 6.1.5. The VACCHCS facility shall identify and provide the records/documents to be scanned, imported, and indexed. The VACCHCS facility requires the contractor will prioritize the scanning of all records stored in boxes and identified as loose files and miscellaneous items such as loose files. These records consist of 3,955 boxes of records located with contractor Sourcecorp/Access Deliverex and are currently coded: - Medical Records\133-Fresno - Medical Records\3639-Fresno - DX Inpatient\3504-Charts 6.1.6. Contractor shall verify that the documents meet specified criteria for scanning or importing. a. Contractor shall follow the facility specified criteria, in accordance with VHA Handbook 1907.01 and VHA Handbook 1907.07 to include checking for the following items: signed document, not a draft, correct type of document, correct patient, etc. 6.1.7. Contractor shall prepare documents for scanning and importing; a. Scanning - Contractor shall perform the following items: remove staples, ensure document is flat, pages are sequenced in order, document is not torn and if so repair it, etc. The Contractor shall complete the task in a manner that ensures they meet the stated performance metrics indicated in the QASP. b. Importing - Contractor shall perform the following items: pages are sequenced in order; pages are readable; page orientation is accurate, etc. The Contractor shall complete the task in a manner that ensures they meet the stated performance metrics indicated in the QASP. 6.1.8. Contractor shall index the documents (e.g., match patient identifiers, date of care, document type, index term) in VistA Imaging. 6.1.9. Contractor shall scan and import the documents in VistA Imaging in accordance with VHA Handbook 1907.01, VHA Handbook 1907.07, VistA Imaging Clinical Capture User Manual, HIM Practice Brief Community Care VistA Imaging Capture and Minimum Documentation Requirements , and HIM Practice Brief HIM Importing From FBCS User Guide . 6.2. Contract staff performing scanning, importing, and indexing functions shall perform 100% quality checks in accordance with VHA Handbook 1907.07 (Paragraph 20. Conducting Quality Assurance Monitors) 6.2.1. Contract staff shall report any identified quality errors immediately to the Contractor manager/supervisor. In turn, the Contractor manager/supervisor shall notify local VA facility POC of quality error for medical record correction. The contractor shall perform random Quality Assurance (QA) monitoring of staff work and report results of QA monitoring to the designated VA representative in quarterly intervals. Any identified errors shall be reported and remediated with the facility POC. 6.2.2 Once contract staff has performed the quality checks in accordance with VHA Handbook 1907.07 the contractor will complete destruction of records within 30 days of receipt of destruction approval from VA Facility Records Officer. Confirmation of destruction will be sent to the VA Facility Records Officer within 15 days of destruction completion. 7. SERVICES NOT INCLUDED IN THE SCOPE OF THIS CONTRACT: It is not expected that special considerations shall be made for diagnostic quality imaging. The digital a. capture of x-rays or other diagnostic media is not within the scope of work. Digital capture of x-rays or other diagnostic media received relating to VACC shall be sent to the Radiology Service. b. Scanning entire volumes of complete inactive records is not part of the contract. c. Scanning/digitizing clinical documents and providing image files in a shared environment as the end and/or interim process is not part of the contract. Contractor shall be required to incorporate/index appropriately the scanned clinical document into VistA Imaging and will not be reimbursed for images stored in a share environment or web repository during this contract period. 8. SPECIFIC TASKS AND DELIVERABLES: The Contractor shall provide, at a minimum, all tasks and deliverables described within this document. All deliverables shall be submitted to the local Ordering Officer (OO) and local Contracting Officer Representative (COR). Unless otherwise stipulated, written deliverables shall be phrased in plain English. Statistical and other technical terminology shall not be used without providing a glossary of terms. 8.1.1. The Contractor shall provide all management, labor, resources, and specialized equipment needed to provide Record Retirement, Physical Relocation of Active Records, Scanning and/or Importing and Indexing of Records, and Project Management. The Contractor shall define a VA-approved project workflow that shall not impede the day-to-day operations of the specific department. When the Contractor cannot meet the specified deadline, they must inform the local facility COR within five (5) business days. If the Contractor is uncertain concerning a policy or procedure, the Contractor shall consult with the local facility COR for clarification. 8.1.2. The Contractor is responsible for the management of its staff, and the Contractor is responsible for training its staff on VA policy and procedures, with the assistance of the Health Information Management (HIM) Department. VACCHCS shall provide the Contractor with a high level of explanation of the file room process including the unique indexing requirements for VistA Imaging. 8.1.3. The Contractor shall bear the costs of any other training. The Contractor is responsible for performing quality reviews of staff work. The Contractor shall bear the cost of any re-work and/or corrections and take corrective action when services are found deficient or substandard. 8.1.4. The Contractor shall also complete standard administrative tasks (e.g., background investigation forms, security training, etc.) as required by the PWS. 9. PROJECT MANAGEMENT: The Contractor shall designate a Project Manager (PM) who shall provide oversight of all contracted efforts. The PM must be available during regular business hours and must return all calls and emails within 24 hours. 10.1.1. The Contractor shall submit quarterly status reports. The Contractor shall make every effort to ensure that issues raised by the local facility are addressed in a timely manner. Work shall not commence until the local CO or designee approves the Project Management Plan. The Contractor shall provide the VA with a Project Management Plan that shall include, but is not limited to: a. Schedule Management The development, management, and control of project schedules. b. Risk Management The planning, identification, analysis, monitoring, and control of and responses to program risk. c. Communication Management The timely and appropriate generation, collection, distribution, storage, retrieval, and disposition of program information. d. Staffing Plan - Details on staffing levels, resources, etc., for both Contractor and Subcontractor personnel. e. Quality Assurance Plan - Detailed quality control and oversight plan for all work performed. This plan shall, at a minimum, address how the Contractor shall do the following: Ensure VA information (paper and electronic) is not compromised. Account for all records/information from the point of receipt through storage off-site; and, Ensure proper indexing of records. 11. SECURITY AND PRIVACY REQUIREMENTS: It has been determined that protected health information may be disclosed or accessed, and a signed Business Associate Agreement (BAA) shall be required. The Contractor shall adhere to the requirements set forth within the BAA, referenced in the contract. 11.1.1. POSITION/TASK RISK DESIGNATION LEVEL(S) Position Sensitivity Background Investigation (in accordance with Department of Veterans Affairs 0710 Handbook, Personnel Suitability and Security Program, Appendix A) Low / Tier 1 Tier 1 / National Agency Check with Written Inquiries (NACI) A Tier 1/NACI is conducted by OPM and covers a 5-year period. It consists of a review of records contained in the OPM Security Investigations Index (SII) and the DOD Defense Central Investigations Index (DCII), Federal Bureau of Investigation (FBI) name check, FBI fingerprint check, and written inquiries to previous employers and references listed on the application for employment. In VA it is used for Non-sensitive or Low Risk positions. Moderate / Tier 2 Tier 2 / Moderate Background Investigation (MBI) A Tier 2/MBI is conducted by OPM and covers a 5-year period. It consists of a review of National Agency Check (NAC) records [OPM Security Investigations Index (SII), DOD Defense Central Investigations Index (DCII), FBI name check, and a FBI fingerprint check], a credit report covering a period of 5 years, written inquiries to previous employers and references listed on the application for employment; an interview with the subject, law enforcement check; and a verification of the educational degree. High / Tier 4 Tier 4 / Background Investigation (BI) A Tier 4/BI is conducted by OPM and covers a 10-year period. It consists of a review of National Agency Check (NAC) records [OPM Security Investigations Index (SII), DOD Defense Central Investigations Index (DCII), FBI name check, and a FBI fingerprint check report], a credit report covering a period of 10 years, written inquiries to previous employers and references listed on the application for employment; an interview with the subject, spouse, neighbors, supervisor, co-workers; court records, law enforcement check, and a verification of the educational degree. 11.1.2. CONTRACTOR PERSONNEL SECURITY REQUIREMENTS a. The Contractor shall prescreen all personnel requiring access to the computer systems to ensure they maintain the appropriate Background Investigation, and are able to read, write, speak and understand the English language. b. The Contractor shall bear the expense of obtaining background investigations. c. Within 3 business days after award, the Contractor shall provide a roster of Contractor and Subcontractor employees to the COR to begin their background investigations in accordance with the ProPath template. The Contractor Staff Roster shall contain the Contractor s Full Name, Date of Birth, Place of Birth, individual background investigation level requirement (based upon Section 6.2 Tasks), etc. The Contractor shall submit full Social Security Numbers either within the Contractor Staff Roster or under separate cover to the COR. The Contractor Staff Roster shall be updated and provided to VA within 1 day of any changes in employee status, training certification completion status, Background Investigation level status, additions/removal of employees, etc. throughout the Period of Performance. The Contractor Staff Roster shall remain a historical document indicating all past information and the Contractor shall indicate in the Comment field, employees no longer supporting this contract. The preferred method to send the Contractor Staff Roster or Social Security Number is by encrypted e-mail. If unable to send encrypted e-mail, other methods which comply with FIPS 140-2 are to encrypt the file, use a secure fax, or use a traceable mail service. d. The Contractor should coordinate the location of the nearest VA fingerprinting office through the COR. Only electronic fingerprints are authorized. e. The Contractor shall ensure the following required forms are submitted to the COR within 5 days after contract award: 1) For a Tier 1/Low Risk designation: a) OF-306 b) DVA Memorandum Electronic Fingerprints 2) For Tier 2/Moderate or Tier 4/High Risk designation: a) OF-306 b) VA Form 0710 c) DVA Memorandum Electronic Fingerprints f. The Contractor personnel shall submit all required information related to their background investigations (completion of the investigation documents (SF85, SF85P, or SF 86) utilizing the Office of Personnel Management s (OPM) Electronic Questionnaire for Investigations Processing (e-QIP) after receiving an email notification from the Security and Investigation Center (SIC). g. The Contractor employee shall certify and release the e-QIP document, print and sign the signature pages, and send them encrypted to the COR for electronic submission to the SIC. These documents shall be submitted to the COR within 3 business days of receipt of the e-QIP notification email. (Note: OPM is moving towards a click to sign process. If click to sign is used, the Contractor employee should notify the COR within 3 business days that documents were signed via eQIP). h. The Contractor shall be responsible for the actions of all personnel provided to work for VA under this contract. In the event that damages arise from work performed by Contractor provided personnel, under the auspices of this contract, the Contractor shall be responsible for all resources necessary to remedy the incident. i. A Contractor may be granted unescorted access to VA facilities and/or access to VA Information Technology resources (network and/or protected data) with a favorably adjudicated Special Agreement Check (SAC) or Closed, No Issues (SAC) finger print results, training delineated in VA Handbook 6500.6 (Appendix C, Section 9), and, the signed Contractor Rules of Behavior. However, the Contractor shall be responsible for the actions of the Contractor personnel they provide to perform work for VA. The investigative history for Contractor personnel working under this contract must be maintained in the database of the Office of Personnel Management (OPM). j. The Contractor, when notified of an unfavorably adjudicated background investigation on a Contractor employee as determined by the Government, shall withdraw the employee from consideration in working under the contract. k. Failure to comply with the Contractor personnel security investigative requirements may result in loss of physical and/or logical access to VA facilities and systems by Contractor and Subcontractor employees and/or termination of the contract for default. l. Identity Credential Holders must follow all HSPD-12 policies and procedures as well as use and protect their assigned identity credentials in accordance with VA policies and procedures, displaying their badges at all times, and returning the identity credentials upon termination of their relationship with VA. 12. PERFORMANCE METRICS Performance Objective Performance Standard Acceptable Levels of Performance A. Timelines of scanning, importing and indexing of documents 1. To provide timely inclusion of scanned/imported and indexed documents into VistA Imaging. 1. 33% of the received hard copy records must be scanned and uploaded into VistA Imaging each year. 2. All records must be scanned and uploaded into VistA Imaging before the end of OY2 3. Once Q/A is complete and record upload verified, hard copies will be by VA for destruction with all destruction completed before the end of OY2 B. Quality and Accuracy of Scanning and Importing of Documents 1. Contractor performs quality assurance reviews of contract scanning staff. Reported reviews to COR must demonstrate at least 95% compliance rate. 1. The quality standard is a 95% compliance rate and frequency of review submission by Contractor. For example, scanned documents reviewed for quality demonstrate accuracy in terms of completeness including patient identification, legibility, indexing and orientation and represent an exact replica of original document. For example, 95/100 documents reviewed demonstrate quality compliance with completeness, legibility, indexing and orientation. C. Effectiveness of Reporting 1. Contractor quality assurance reviews of contract scanning staff must be received in a timely manner no later than the 15th of the month following the end of the previous quarter. 2. All boxes for retirement and relocation are prepared appropriately (taped shut, correct box number on box, correct accession number on box. 1. VA designee shall pull a 5% random sample of records to be retired and relocated. To assure the same records on the pull list are in the boxes, the pull list must match the box record content 100% of the time. 2. All boxes reviewed by VA designee and boxes must be prepared appropriately 100% of the time. Note 1: Significant errors or omissions are defined as deliverables not meeting the intent of the contract and the work to be considered to be within scope of the vehicle. Note 2: Continued repetitive errors may result in an unacceptable rating on performance report to be used as a part of evaluation criteria on future competition 12.1.1. The COR shall utilize a Quality Assurance Surveillance Plan (QASP) throughout the life of the contract to ensure that the Contractor is performing the services required by this PWS in an acceptable level of performance. The Government reserves the right to alter or change the surveillance methods in the QASP at its own discretion. A Performance Based Service Assessment shall be used by the COR in accordance with the QASP to assess Contractor performance. 13. FACILITY PROVISIONS: The Contractor shall request other Government documentation deemed pertinent to the work accomplishment directly from the Government officials with whom the Contractor has contact. The Contractor shall consider the COR as the final source for needed Government documentation when the Contractor fails to secure the documents by other means. The Contractor is expected to use common knowledge and resourcefulness in securing all other reference materials, standard industry publications, and related materials that are pertinent to the work. 13.1.1. VA may provide remote access to VA specific systems/network in accordance with VA Handbook 6500, which requires the use of a VA approved method to connect external equipment/systems to VA s network. Citrix Access Gateway (CAG) is the current and only VA approved method for remote access users when using or manipulating VA information for official VA Business. VA permits CAG remote access through approved Personally Owned Equipment (POE) and Other Equipment (OE) provided the equipment meets all applicable 6500 Handbook requirements for POE/OE. All of the security controls required for Government-Furnished Equipment (GFE) must be utilized in approved POE or OE. The Contractor shall provide proof to the COR for review and approval that their POE or OE meets the VA Handbook 6500 requirements and VA Handbook 6500.6 Appendix C, herein incorporated as Addendum B, before use. CAG authorized users shall not be permitted to copy, print or save any VA information accessed via CAG at any time. VA prohibits remote access to VA s network from non-North Atlantic Treaty Organization (NATO) countries. The exception to this are countries where VA has approved operations established (e.g. Philippines and South Korea). Exceptions are determined by the COR in coordination with the Information Security Officer (ISO) and Privacy Officer (PO). 13.1.2. This remote access may provide access to VA specific software such as Veterans Health Information System and Technology Architecture (VistA), VistA Imaging, and Third-Party Document Management Software. The Contractor shall utilize Government provided software development and test accounts, document and requirements repositories, etc. as required for the development, storage, maintenance and delivery of products within the scope of this effort. The Contractor shall not transmit, store or otherwise maintain sensitive data or products in Contractor systems (or media) within the VA firewall IAW VA Handbook 6500.6 dated March 12, 2010. All VA sensitive information shall be protected at all times in accordance with VA Handbook 6500, local security field office System Security Plans (SSP s) and Authority to Operate (ATO) s for all systems/LAN s accessed while performing the tasks detailed in this PWS. The Contractor shall ensure all work is performed in countries deemed not to pose a significant security risk.
