PERFORMANCE WORK STATEMENT (PWS) DEVELOP FACILITY MAINTENANCE MANAGEMENT PLAN Buildings 100 G-wing, 500, 603, 615 & 710 March 30, 2023 OBJECTIVE The purpose of this acquisition is to provide the Department of Veterans Affairs, Veterans Health Administration (VHA) with a maintenance management plan for the Palo Alto Buildings 100 G-wing, 500, 603, 615 and 710. The objective is to develop a maintenance management plan for periodic and preventative maintenance within the facility maintenance system in VistA. BACKGROUND The Department of Veterans Affairs (VA) has recently completed construction on three buildings that require regular periodic maintenance. The Building 100 G-wing is 35,000 GSF. The Polytrauma Building is 175,000 GSF (Building 500). The Generator Building (Building 603) is 4500 GSF. The Chiller Building (Building 615) is 3600 GSF. The Research/VMU Building (Building 710) is 95,700 GSF. SCOPE The Contractor shall develop a maintenance management plan for the following buildings: the first floor and basement of Building 100 G-wing; the Polytrauma Building 500; the Generator Building 603; the Chiller Building 615; and the Research/VMU Building 710 at the VA Palo Alto Health Care System at 3801 Miranda Avenue, Palo Alto, California 94304. The Contractor shall obtain and compile all necessary information from site visits, as-builts, shop drawings and any other pertinent documentation to develop the maintenance management plan. For reference, see section 37 for a list of Drawing and Specification attachments for each building. The equipment and systems to be maintained in Building 100 G-wing are defined in the Centers for Ambulatory Care, Polytrauma & Blind Rehabilitation Radiology Consolidation Table of Contents specification. The equipment and systems to be maintained in Building 500 are defined in the Centers for Ambulatory Care, Polytrauma & Blind Rehabilitation Polytrauma & Blind Rehabilitation Table of Contents specification. See Attachment 2 in section 37. The equipment and systems to be maintained in Buildings 603 and 615 are defined in the Centers for Ambulatory Care, Polytrauma & Blind Rehabilitation - Loop Road and Site Utilities, Radiology Consolidation, Parking Structure 2 Table of Contents specification. The equipment and systems to be maintained in Building 710 are defined in the Centers for Ambulatory Care, Polytrauma & Blind Rehabilitation - Research and VMU Table of Contents specification. CHANGES TO THE PERFORMANCE WORK STATEMENT Any changes to this PWS shall be authorized and approved only through written correspondence on a Standard Form (SF-30) by a Contracting Officer (CO). A copy of each change and resulting contract modification shall be kept in a project folder along with all other products of the project. Costs incurred 10 by the Contractor through the actions of parties other than the CO shall be the total responsibility of the Contractor and no additional cost shall be charged to the Government. *Please reference Performance Work Statement (PWS) for additional information
