Ensure to send in a signed memo stating that you can handle whats described in the PWS. Sign the 1st page of the SF1449, blocks 30a thru 30c then fill out the price cost schedule from pages 5 to 6. Sign the QASP as well. This is for SUD recovery treatment services. Also, pay attention to the addendum of instructions within the RFQ. Everything must be turned in by the due date of 1 Aug 23.
