Type of Service Continue with utilization of the allocated services previously made available by the Oahu Veterans Council (OVC) that support weekly Gero-Fit classes Location/Point of Contact Venue should be within a 10-mile radius of the Center for Aging due to need to transport equipment between the main medical center and the off-site location. The Center for Aging is located at the following address: Spark M. Matsunaga Department of Veterans Affairs Medical Center 459 Patterson Road Honolulu, HI 96819-1522 Point of Contact: Gero-Fit Program Coordinator Michelle McDonald Contact #: 808-433-7848 Email: Michelle.McDonald@va.gov Period of Performance One base year with four option years included Base Year: 02/01/2023-01/31/2024 Option Year 1: 02/01/2024-01/31/2025 Option Year 2: 02/01/2025-01/31/2026 Option Year 3: 02/01/2026-01/31/2027 Option Year 4: 02/01/2027-01/31/2028 If the services after the base year or option year are no longer deemed necessary, then the final payment will be equivalent to the total months of services consumed. Description of Service The venue will operate as a functionable site for weekly Gero-Fit services to be conducted on Mondays, Wednesdays, and Fridays excluding Federal Holidays. The Gero-Fit program would need to be allocated a specific hall or location to conduct their classes Specific services that will be utilized include: Air conditioning, public Wi-Fi, smart TV for hybrid classroom activities (virtual and face-to-face), electricity, use of chairs and tables, designated hall and storage space, bathrooms, janitor cleaning, and assigned parking spaces There will be 3 classes/groups each day, with class size ranging from 5-10
