This is not a solicitation for bids, proposals, proposal abstracts, or quotations. The purpose of this Sources Sought Notice is to obtain information regarding the availability and capability of all qualified sources to perform a potential requirement. The responses received from interested contractors will assist the Government in determining the appropriate acquisition method. The Department of Veterans Affairs (VA), NCO 21- is conducting a market research survey to identify potential qualified sources capable of providing direct shipment of medical and surgical supplies for VA Palo Alto Health Care System Pharmacy (Station 640), and prescribed Veteran patients homes located within Station 640 s area of responsibility with addresses supplied. The contract will serve as a back-up in order to prevent missed deliveries to patients and facilities when items are out of stock or not available from the Consolidated Mail Outpatient Pharmacy (CMOP). Period of Performance: Five (5) Years. RESPONSES MUST INCLUDE THE FOLLOWING INFORMATION: Company Name, Address, Size Standard, UEI#, Cage Code Point of Contact name, phone number, and email address NAICS Code 339113 and Small Business Size Standard 750 employees Statement of Capability that demonstrates the offeror's past performance in providing this type of service.
