The purpose of this Sources Sought Notice is to obtain information regarding the availability and capability of all qualified sources to perform a potential requirement. The responses received from interested contractors will assist the Government in determining the appropriate acquisition method. The Department of Veterans Affairs (VA), NCO 21- is conducting a market research survey to identify potential qualified sources capable of providing Hemostasis Test Instrumentation and supporting reagents and supplies Cost-Per-Reportable-Results at a fixed price for a period of performance of 5 years at VA Palo Alto Health Care System at 3801 Miranda Ave, Palo Alto, CA 94304. See attached DRAFT Statement of Work. RESPONSES MUST INCLUDE THE FOLLOWING INFORMATION: Company Name, Address, Size Standard, UEI#, Cage Code Point of Contact name, phone number, and email address NAICS Code 334516 and Small Business Size Standard 1,250 employees Statement of Capability that demonstrates the offeror's past performance in providing this type of supplies. Place of Manufacture Contract Number for any existing contracts that order can be made from Estimated amount of time needed for laboratory analyzer transition (If applicable) Responses are due to Mohammed F Hussain at mohammed.hussain2@va.gov by 05:00PM Pacific Time on Wednesday, August 31, 2022. No submissions will be accepted after this date and time. This is a Sources Sought Notice and submissions will be used for informational and planning purposes only.
