2 STATEMENT OF WORK Biolase Waterlase Express Background: Dental Service is attempting to procure two Biolase Waterlase express units which are all-tissue laser that uses laser energy and waterpower to conduct dental procedures while relieving dental anxiety in patients. The units possess an animated step-by-step guide, customer care support, and online training access with the flexibility and access required for in-depth dental procedures. Scope/Statement of Work: The current Biolase laser units are inoperable, and the costs associated with repair far exceed the benefit of total replacement. Having an outdated unit makes it hard to repair and replace parts that are needed as well as ensures an impossibility for software upgrades to take place. This solicitation is with the intent of replacing the existing units and receiving trade-in value for the financial benefit of the federal government. Training: Must include on-site training for all dental providers, clinicians, and Biomedical Engineers. Must have the option to perform training during a live procedure. The vendor must agree to the privacy and security requirements of the facility including Business Associate Agreement if required. Market Research VASNHS Chief Dental Service completed a search and found the Biolase not sold by multiple vendors to be an acceptable system based on exams performed at Dental Services by technologists in the VASNHS. The Biolase Waterlase Express is an upgrade from the current Laser system that was in use prior to failure. Instructors presenting the training shall be fully licensed and qualified to perform the training. The training shall be instructor-led training and shall be in English. User training shall take place at VASNHS Dental Service. Training shall be customized to meet the training requirements of VASNHS. Training shall cover basic and advanced application topics, including, but not limited to, applications, system workflow, interface, and buttonology. The training objectives will be finalized by the VA POC prior to the start of training. All training materials shall be in English, and they may include training manuals, notebooks, compact disks, and/or online access to information. Training shall be scheduled within forty-eight (48) hours following the award of the contemplated contract. The name and contact information of the VA point of contact (POC) shall be provided to the contractor upon award of the contemplated contract. The specific date for training shall be agreed upon by the VA POC and contractor. The training will be scheduled for the earliest date possible. Training shall be conducted during normal work hours from 7:30 a.m. 4 p.m. Pacific Standard Time during the workweek. A workweek includes Monday, Tuesday, Wednesday, Thursday, and Friday. The contractor and contractor s personnel shall be subject to the laws, regulations, standards, VA Directives, VA Handbooks, VA Memos, and VA SOPs as VA personnel. The contractor shall request logical (technical) or physical access to VA information and/or VA information systems for their employees only to the extent necessary to perform the services specified in this contract. All contractor personnel are required to wear a time-limited, VA identification (ID) badge at all times while at the VASNHS facility. The contractor s employees must have a valid state driver s license or state identification card to obtain a VA ID badge. ID badges shall be obtained from the VASNHS police department. All contractor personnel are required to sign in and out at the VA police dispatch at the corresponding VASNHS locations as directed by the VA POC.
