THIS IS A SOURCES SOUGHT NOTICE ONLY. This is not a solicitation for bids, proposals, proposal abstracts, or quotations. The purpose of this Sources Sought Notice is to obtain information regarding the availability and capability of all qualified sources to perform a potential requirement. The responses received from interested contractors will assist the Government in determining the appropriate acquisition method. The Department of Veterans Affairs (VA), NCO 21- is conducting a market research survey to identify potential qualified sources capable of providing the BRAND NAME ONLY Get Well Outpatient Digital Care Management System at VA Northern California Health Care System at 10535 Hospital Way Mather, CA 95655. VA Northern California Health Care System currently has the Get Well Inpatient Digital Care Management System currently installed and is looking to expand to outpatient services. ***See attached for a DRAFT Statement of Work*** RESPONSES MUST INCLUDE THE FOLLOWING INFORMATION: Company Name, Address, Size Standard, UEI#, Cage Code Point of Contact name, phone number, and email address NAICS Code 541519 and Small Business Size Standard $30.0M Authorized Distributor letter (If applicable) Responses are due to Moneque Rodriguez at moneque.rodriguez@va.gov by 15:00AM Hawaii Time on Friday, July 08, 2022. No submissions will be accepted after this date and time. This is a Sources Sought Notice and submissions will be used for informational and planning purposes only. Respondents are solely responsible for all expenses associated with responding to this RFI.
