Presolicitation Notice Presolicitation Notice Page 3 of 3 Presolicitation Notice *=Required Field Presolicitation Notice Page 1 of 3 This is a notice of intent to sole source a contract under the authority of FAR 13.106- 1(b)(2), only one source being available to meet the Government s need. The contract will be for the base period. The North American Industrial Classification System (NAICS) code for this procurement is 334510: Electromedical and Electrotherapeutic Apparatus Manufacturing, with a small business size standard of 1,250. The FSC/PSC is 6515: Medical and Surgical Instruments, Equipment, and Supplies. The Department of Veterans Affairs intends to solicit and award a sole source contract for the Verifit 2 Audioscan at Palo Alto VA Medical Center -Menlo Park to E3 Diagnostics, Inc., 3333 N Kennicott Ave, Arlington Heights, IL 60004-1429. There are no other source of supply/service that will satisfy agency requirements. E3 Diagnostics, Inc., 3333 N Kennicott Ave, Arlington Heights, IL 60004-1429 is the sole vendor, and no other business is authorized to sell or distribute their products. E3 Diagnostics, Inc is the only source capable of providing the Verifit 2 Audioscan. There are no other companies that were identified that can meet this requirement. The Palo Alto VA Medical Center -Menlo Park is seeking to establish the following: Verifit Part Number Description Qty 3547 Audioscan Verifit 2 Clinical REM/HIT - replaces ETY-VERIFIT 2 FSS Pricing for VA Contract 10 VA-201 Audioscan Verifit 2/Axiom NOAH Module License 1 VA-201-Addtl Audioscan additional NOAH Module 9 N/A Training 1 Any responsible source that feels it is capable in performing this requirement may submit a quote (monthly) for the above listed items and describe and/or submit documentation to support your capabilities. The response will be considered by the agency. If you are not the Original Equipment Manufacturer (OEM), you must submit an authorized distributor letter signed by the OEM. If you are not the OEM and a small business, you must indicate in your response how you will comply with limitations on subcontracting and/or the non-manufacturer rule. Responses should be submitted to Alex Hernandez at Alex.Hernandez@va.gov no later than June 17, 2022, at 15:00 PM PT to be considered. If no responses are received, the Government will continue with its sole source procurement, and a sole source Request for Quote will be published to contract opportunities in accordance with FAR 5.102
