Description This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Federal Acquisition Regulation (FAR) subpart 12.6, Streamlined Procedures for Evaluation and Solicitation for Commercial Items, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; quotes are being requested, and a written solicitation document will not be issued. This solicitation is issued as an RFQ. The solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 2022-03 Effective 01-01-2022. This solicitation is set-aside for 100% Small Business (SB). The associated North American Industrial Classification System (NAICS) code for this procurement is 541690, with a small business size standard of $16,500,000. The Product Service Code (PSC) is H365. The VA Central California Healthcare System located at 2615 E Clinton Ave, Fresno, CA 93703 is seeking to purchase on-site diagnostic medical physics support services. The services include inspecting all imaging equipment, perform acceptance testing, inspect all imaging equipment after repairs, review CT protocols, provide shield design calculations for each new, replaced, or relocated x-ray imaging system, assist in the development of a comprehensive technical quality assurance program, review reports form Engineering to verify compliance of any necessary corrective actions, and test radiographic imaging systems. The purchase order period of performance is as follows: Base period: 03/01/22 02/28/23 Option period 1: 03/01/23 02/29/24 Option period 2: 03/01/24 02/28/25 Option period 3: 03/01/25 02/28/26 Option period 4: 03/01/26 02/28/27 Place of Performance: Work will be performed at the VA Central California Healthcare System, 2615 E.
