This announcement constitutes the only solicitation; quotes are being requested, and a written solicitation document will not be issued. This solicitation is issued as an RFQ. The solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 2021-05 Effective 03-10-21. This solicitation is set-aside for Veteran-Owned Small Businesses (VOSB). In accordance with 38 U.S.C. 8127-8128 as implemented by VAAR 819.7001(a), VOSBs must have their ownership and control verified by VA and listed in the Vendor Information Pages (VIP) at the time of submission of quotes and prior to award of any contracts. The Product Service Code (PSC) is H365. The VA Southern Nevada Healthcare System (VASNHCS) located at 6900 North Pecos Rd, N. Las Vegas, NV 89086 is seeking to purchase diagnostic radiation physics consulting services to the radiology department. The services include weekly on-site visits for the support of all aspects of radiation safety, servicing of radiology equipment, licensing and meeting FDA standards, and serving as the Radiation Safety Officer. The purchase order period of performance is as follows: Base period: 09/01/21 08/31/22 Option period 1: 09/01/22 08/31/23 Option period 2: 09/01/23 08/31/24 Option period 3: 09/01/24 08/31/25 Option period 4: 09/01/25 08/31/26 Place of Performance Work will be performed at the VA Southern Nevada Healthcare System (VASNHCS) in the Radiology department and four (4) Primary Care Clinics (PCC).
