The Department of Veterans Affairs (VA) San Francisco Community Based CBOCs require temporary Locum Primary Care Physician services at the San Francisco VA Health Care System’s (SFVAHCS) Community Based Outpatient Clinics (CBOCs). The Locum Physician(s) shall provide ongoing primary care services for chronic medical problems commonly seen in the veteran population; included are such disorders as diabetes, hypertension, chronic obstructive pulmonary disease, degenerative arthritis, depression, congestive heart failure, angina and pain. Judicious use of laboratory and x-ray services as well as appropriate referral and consultation is obtained in accordance with the terms and conditions contained herein. Period of Performance: Base year plus 4 option years from 1/1/2021 to 10/31/2026 Base period: November 1, 2021 through October 31, 2022 Option Year 1: November 1, 2022 through October 31, 2023 Option Year 2: November 1, 2023 through October 31, 2024 Option Year 3: November 1, 2024 through October 31, 2025 Option Year 4: November 1, 2025 through October 31, 2026
